急性脑梗死不同方式动脉机械性溶栓的临床治疗探讨

目的 通过对118例急性脑梗塞患者进行不同方式机械性动脉溶栓的治疗,就其治疗方法、时间窗、溶栓药物的剂量及并发症进行探讨.方法 选择急性发病在12h内的脑梗死病人118例,采用经不同方式机械性动脉溶栓,通过脑血管造影判断其血管开通情况并对其治疗疗效及并发症进行评估.结果 118例急性脑梗塞患者中血管完全再通率57.6％(68/108);部分再通率为34.7％( 41/118);未开通9例7.7％;临床治愈76例占64.4％;显著好转20例占16.95％,有效14例占11.9％;无效7例占5.9％;死亡1例占0.85％.结论 经动脉不同方式机械性介入溶栓是治疗急性脑梗死的一种安全有效的方法.建立急救绿色通道和规范其治疗措施,能较好的提高急性脑梗死患者救治的成功率,降低死亡率和致残率.

Mechanical intra-artera thrombolysis therapies of patients with acute cerebral infarction

【Objective】 To investigate the treatment time window,the thrombolytic drug dose and complications in 118 patients of acute cerebral infarction treated with different mechanical intra-artera thrombolysis therapies.【Methods】 All 118 acute cerebral infarction patients admitted within 12 hours,were treated by different mechanical intra-artera thrombolysis therapies.The therapeutic effect was evaluated by recanalization through cerebral angiography.【Results】 The completely recanalization rate,partial recanalization rate and non recanalization rate was 57.6%(68/118),34.7%(41/118),7.7%(9/118);76(64.4%) case were clinical cure,20(16.95%) obviously,improved 14(11.9%),partially improved 7(5.9%),invalid 1(0.85%) dead cases in our study.【Conclusion】 The treatment of acute cerebral infarction by different mechanical intra-artera thrombolysis therapies is safe and effective.The cure rate will be improved by setting up first aid system and standard therapeutic measure.
