特大面积烧伤伴重度吸入性损伤的救治

目的 总结特大面积烧伤伴重度吸入性损伤的治疗经验。方法 回顾分析本科2000~2003年收治的18例特大面积烧伤伴重度吸入性损伤患者的临床资料。结果 18例患者中治愈15例,死亡3例,治愈率83.3%。结论 综合运用各项治疗措施,如早期行预防性气管切开,积极气道灌洗、湿化及应用沐舒坦治疗,早期行肺保护性通气、应用生长激素等,可提高此类患者的治愈率。

Management of extensive burn accompanying severe inhalation injury

Objective To review our clinical experiences in the treatment of extensive burn accompanied by severe inhalation injury. Methods A retrospective analysis was conducted of 18 such cases admitted from 2000 to 2003 in light of the curative rate, mortality rate and measures for effective management. Result and Conclusions Fifteen patients (83.3%) were cured and 3 (16.7%) died. The curative rate of extensive burn accompanied by severe inhalation injury can be enhanced by combining various treatments, including early preventive tracheostomy, appropriate tracheobronchial lavage, humidification and application of ambroxol; protective lung ventilation, and administration of growth hormones should be done as early as possible.
