腹腔镜治疗坏疽性或化脓性阑尾炎合并阑尾根部穿孔的探讨

目的探讨腹腔镜治疗坏疽性或化脓性阑尾炎合并阑尾根部穿孔的疗效及术中处理方法。方法总结2002年12月至2006年7月实施的腹腔镜治疗坏疽性或化脓性阑尾炎合并阑尾根部穿孔的病例68例。分析临床疗效及术中处理方法。结果全组68例,均采用腹腔镜治疗,除1例出现阑尾残端漏外,余患者愈合良好,未出现切口感染、粘连性肠梗阻、腹腔感染等并发症。结论随着腹腔镜技术的不断成熟、完善,对于腹腔镜治疗坏疽性或化脓性阑尾炎合并阑尾根部穿孔的病例也可在不明显增加手术并发症的情况下安全地实施,并突出腹腔镜治疗的优点。

A study on the indication of laparoscopic mueopurulent and gangrenous appendectomy with perforative base

Objective To investigate the therapeutic results and surgical methods of laparoscopic mueopurulent and gangrenous appendectomy with perforative base.Methods 68 cases of laparoscopic mueopurulent and gangrenous appendectomy with perforative base were reviewded restrospectively from December 2002 to july 2006.Results laparoscopic mueopurulent and gangrenous appendectomy with perforative base was used in 68 casse had no sever adhesion and other complications but one case had a leakage.Conclusion With the technical improvement of the laparoscopic appendectomy,laparoscopic appendectomy can be performed safely and effectively and have its advantages with no increase of surgical morbidity rate.
