重度烧伤病人下呼吸道感染病原学分析

摘    要:目的了解重度烧伤患者下呼吸道感染的病原菌分布及其耐药特点,为治疗用药提供依据。方法对2004年3月-2007年2月确诊的重度烧伤并下呼吸道感染的患者分离到的病原菌进行鉴定,并采用纸片扩散法进行药敏试验。结果病原菌中革兰阴性菌占56.82%,以铜绿假单胞菌、鲍氏不动杆菌和肺炎克雷伯菌为主,主要对亚胺培南、头孢吡肟敏感;革兰阳性菌主要是金黄色葡萄球菌,占36.36%,大多为耐甲氧西林金黄色葡萄球菌(MRSA),对万古霉素100%敏感。结论革兰阴性菌及革兰阳性菌均可引起重度烧伤患者下呼吸道.感染,以革兰阴性菌为主,两类病原菌均对多种抗菌药物耐药。

Abstract:Objective To investigate the distribution and drug resistance of pathogenic bacteria of lower respiratory tract infection of severe burn patients. Methods Antimicrobial susceptibility of pathogenic bacteria isolated from lower respiratory tract infection of serier burn patients from Mar 2002 to Feb 2007 was performed by Kirby-Bauer method. Results The main pathogens of lower respiratory tract infection of serier burn patients were Gram-negative bacilli(56.82% ), the most common were staphylococcus aureus, Escherichia coli and Klebsiella puemoniae. The Grampositive organisms ( 36.36% ). The Gram-positive organism was meticillin-resistant S aureus( MRSA). All straina of MRSA were antimicrobial sensitive to vancomycin. Conclusion Gram-negative bacilli and Gram-positive organism are both the pathogenic bacteria of lower respiratory tract infection of severe burn patients. They are highly resistant to the most antimicrobial agents.
