老年2型糖尿病患者尿液白蛋白水平与糖代谢指标的关系探讨

目的：探讨老年2型糖尿病（T2DM）患者尿液白蛋白水平与糖代谢指标之间的关系。方法选择2015年1月—2016年5月我院诊治的老年T2DM患者350例，测定尿白蛋白肌酐比值（UACR）、空腹血浆葡萄糖（FPG）、糖化血红蛋白（HbA1c）和餐后2 h葡萄糖（2 hPG）。结果大量白蛋白尿组（UACR≥300 mg/g）患者FPG、HbA1c和2hPG均高于微量白蛋白尿组（30≤UACR<300 mg/g）患者水平，微量白蛋白尿组患者FPG、HbA1c和2hPG均高于正常白蛋白尿组（UACR<30 mg/g）患者水平（P<0.05）。196例FPG<7.0 mmol/L的患者微量白蛋白尿和大量白蛋白尿发病率均显著低于154例FPG≥7.0 mmol/L患者（P<0.05）。203例HbA1c<6.5%患者微量白蛋白尿和大量白蛋白尿发病率均显著低于147例HbA1c≥6.5%患者（P<0.05）。结论老年T2DM患者尿液白蛋白水平与FPG、HbA1c和2hPG含量呈正相关。

Relation between urinary albumin levels and glucose metabolism indexes in elderly patients with type 2 diabetes

Objective To investigate the relationship between urine albumin level and the sugar metabolic indices in the elderly patients with type 2 diabetes. Methods 350 cases of elderly patients with type 2 diabetes mellitus (T2DM) in our hospital from January 2015 to May 2016 were employed for research objects. Urinary albumin creatinine ratio (UACR), fasting plasma glucose(FPG), glycosylated hemoglobin(HbA1c) and postprandial glucose(2hPG) were determined. Results FPG, 2hPG and HbA1c in a large number of albuminuria group (UACR≥300 mg/g) were higher than that in patients with microalbuminuria group, and FPG, 2hP G and HbA1c in microalbuminuria group(30≤UACR<300 mg/g) were higher than that in the patients with normal albuminuria group (UACR<30 mg/g)(P<0.05). 196 cases of patients with FPG<7.0 mmol/L microalbuminuria and a large number of albuminuria morbidity were significantly lower than that of 154 cases of FPG≥7.0 mmol/L patients (P<0.05). In 203 cases of diabetes with HbA1c<6.5%, the incidence of microalbuminuria and a large number of albuminuria were significantly lower than that in 147 patients with HbA 1c ≥6.5% (P<0.05). Conclusion The urine albumin levels were positively correlated with the content of FPG , 2hPG and HbA1c in elderly patients with T2DM.
