肺叶切除作为一种肺减容手术新方法

目的：寻求一种简单易行的肺减容手术新方法治疗严重慢性阻塞性肺疾病（ＣＯＰＤ），探讨严重ＣＯＰＤ病人对肺叶切除耐受能力及手术之后对呼吸功能的影响。方法：对１１例严重ＣＯＰＤ并发肺肿瘤和５例单纯性严重ＣＯＰＤ病人实施肺叶切除或多边肺切除。对比围手术期内二种手术方法的优缺点。并对病例进行了术后随诊，对比手术前和随访时多项呼吸功能指标。结果：肺叶切除与多边肺切除相比，术后并发症少，基本上不存在肺漏气问题，术后拔管和住院时间较短，且同样能达到术后远期呼吸功能改善的目的。结论：肺叶切除作为一种肺减容手术新方法，简单易行，术后并发症少，值得推广。

Lobectomy as a new way of lung volume reduction surgery

Objective:To find out a new simple way of lung volume reduction surgery (LVRS) for severe chronic obstructive pulmonary disease (COPD),and to study the tolerance to lobectomy in the patients with severe COPD and the effects on respiratory function postoperatively.Methods:Lobectomy or multiple wedge resection was performed in eleven lung tumor patients with severe COPD and five patients with pure severe COPD.The advantage of two ways mentioned above was compared in the perioperative period.Followup survey was made after the oparation.Preoperative and followup respiratory multiple indices were compared.Results:There were less complications,shorter time of decannulation and stay in hospital and no lung leakage basically after operations in lobectomy than in multiple wedge resection.Lobectomy improved respiratory function at a specified future date as well as multiple wedge resection.Conclusion:Lobectomy served as a new way of LVRS for severe COPD which is simple and convenient,has less complications an
