380例冠心病的心室晚电位观察

摘    要:目的 探讨冠状动脉粥样硬化性心脏病 (冠心病 )患者心室晚电位 (VLP)各项异常指标的发生率及临床价值。方法 对 380例冠心病患者结合临床各项资料并与 80例无冠心病者对比 ,进行VLP结果分析。结果 对照组无VLP(+) ,5例为 (± ) (6 2 5 % )。冠心病组 139例VLP(+) (36 5 8% ) ,6 8例 (± )(17 89% ) ,两组差异显著 (P <0 0 5 )。有心肌梗死病史者中 2 9例进行了随访 ,其中 10例VLP转为异常之后 ,6例发生急性心肌梗死 ,2例合并充血性心力衰竭 ,2例合并室壁瘤。有 6例VLP异常转轻 ,其中 3例为急性心肌梗死后恢复期。结论 冠心病及心肌梗死者VLP(+)检出率明显高于其他疾病。随冠心病的病程变化 ,VLP相应出现动态改变。不论VLP(+/ - )对冠心病均有价值。定期随访可提高VLP预测的准确率。

Abstract:Objective To inquire various kinds of VLP abnomal rates and clinical value.Methods The clinical material of 380 cases with coronary heart were contrasted to those of 80 cases who had non coronary heart disease for VLP analysis.Results Control group had no VLP(+),5 cases were (±)(6.25%).The group which suffer from coronary heart disease had 139 cases VLP(+)(36.58%),68 cases were (±)(17.89%),the positive rate of VLP had obvious difference(P<0.05).We do follow up for 29 patients with heart infarction,there are 10 cases VLP became abnormal,later then 6 cases suffer from acute myocardial infarction,2 cases suffer from congestive heart failure,2 cases have ventricular aneurysm.And then,6 cases VLP abnormal became slight,3 cases with acute myocardial infarction came to recovery stage.Conclusion The positive rate of VLP coronary heart disease and myocardial infarction is higher obviously than that of other diseases.And VLP changes with coronary heart disease's course change.No matter VLP(+/-) is really valuable for coronary heart disease,follow up can increase the accuracy of VLP forecast.
