超声测量胎儿腹围、小脑横径预测胎儿体重的研究

摘    要:研究超声测量胎儿腹围 (AC)、小脑横径 (TCD)预测胎儿体重(FW)的可行性。方法 :应用B超测量 30 0例胎儿的AC、TCD、双顶径 (BPD)及股骨长度 (FL) ,并与新生儿出生体重进行相关分析 ,经二元线性回归导出AC、TCD预测FW的方程式。并对 4 0 0例胎儿用此方程式进行临床验证。结果 :AC与出生体重的相关性最好 ,TCD次之 ,其简单相关系数分别为 0 86435(P <0 0 0 1)、0 61718(P<0 0 0 1) ;偏相关系数分别为 0 78955(P <0 0 0 1)、0 162 90 (P <0 0 0 1)。以这二项指标预测FW的方程式为 :FW =190 .6×AC +332 8×TCD - 4 4 91 5,经 4 0 0例临床验证 ,符合率达 86 5% ,相对误差 <10 %者占 92 5%。结论 :超声测量胎儿AC、TCD预测FW ,方法简便 ,且较准确 ,有较好的临床实用价值。

Abstract:Objective:To study on the ultrasonic measurement of fetal abdominal circumference(AC)and transverse cerebellar diameter(TCD)for the estimation of fetal weight.Methods:The relationship of fetal AC、TCD、biparietal diameter (BPD) and femur length(FL)with neonatal birth weight was studied in 300 cases.Regression analyses was performed to generate the equation of double factors for estimating fetal weight.Prospective study of the effect of the generated equation in estimation of fetal weight was performed in 400 cases.Results:The best correlation was between AC and birth weight,while TCD was the second.The regression equation using AC and TCD was established.The prospective study in 400 cases showed the predictive value of the equation was 86.5%.Conclusions:The ultrasonic measurement of fetal AC and TCD is simple and valuable for estimation of fetal weight.
