脊髓灰质炎后遗成年人爬行—蹲移的外科治疗

目的:探讨脊髓灰质炎后遗成年人爬行——蹲移型的外科治疗策略。方法:治疗14例爬行蹲移患者,男5例,女9例,平均年龄27.5岁。双下肢肌肉重度瘫痪伴多关节畸形,无站立行走功能。术前设计正确的手术方案,应用组合性手术,一期恢复一侧下肢的持重力线,获得良好康复后再实施二期或三期手术治疗。本组病人平均每例实施3人次手术,共实施手术75术次。结果:14例患者术后平均随访3年4个月,全部达到站力行走,其中2例达到徒手短距离行走,12例扶拐行走,全部患者未发生严重并发症。结论:脊髓灰质炎后遗成年人爬行——蹲移患者,只要手术适应证选择得当,整体手术方案制定正确,应用组合性手术,可以获得站立行走的效果。

Surgical Treatment of Adult Greep-Squatting due to Sequela of Poliomyelitis

Objective: To investigate the surgical treatment tactics of adult creep-squatting due to sequela of poliomyelitis. Methods: Fourteen patients with crawl-squatting, male 5 cases, female 9 cases, the mean age of 27.5 years old. All could not stand and walk, the muscle of two legs were serious paralysis and many joints were deformity. The combination operation plans were designed before operation. The weight line of the single leg was fristly recovered, the second or third operation was carried out after getting good recovery. Each patient was averagely operated three times, the number of total operation were 75 times. Result: Fourteen cases were followed up, mean 3 years and 4 months. All patients could stand and walk, 12 cases walked with a limp, 2 cases had no crutch in short distance, no serious complications occurred. Conclusion: The adult patients with creep-squatting due to sequela of poliomylitics have serious and complicated deformity, demanded well operation. As long as choosing proper surgical plans and combined operation, the good results can be obtained.
