冠心病合并糖尿病患者Apelin-12水平及相关性研究

目的分析冠状动脉粥样硬化性心脏病(冠心病)不同程度病变及合并糖尿病患者Apelin-12水平及相关性。方法连续纳入2011年2月至2013年2月在安徽省合肥市第二人民医院心血管内科住院并行冠状动脉造影(CAG)检查的患者195例,男性107例,女性88例,年龄34~87岁。按照CAG检查结果及是否合并糖尿病分为三组:正常组(n=59)、冠心病组(CHD组,n=75)、冠心病合并糖尿病组(CHD+DM组,n=61)。根据CAG结果分为对照组(n=53)、单支组(n=58)、双支组(n=36)和多支组(n=48)。根据CAG结果行Gensini评分。检测血清Apelin-12水平,与冠状动脉病变程度进行相关性分析。结果对照组、单支组、双支组、多支组Apelin-12水平(4.35±1.27)ng/ml vs.(3.36±0.84)ng/ml vs.(2.28±0.33)ng/ml vs.(1.69±0.48)ng/ml]呈下降趋势,Gensini积分(10.63±4.16)vs.(25.64±8.53)vs.(47.18±16.32)vs.(65.18±18.43)]呈升高趋势,差异具有统计学意义(P均0.05)。冠状动脉病变Gensini积分和Apelin-12相关分析结果显示,Gensini积分和Apelin-12水平呈负相关(r=-0.338,P0.05)。结论冠心病患者Apelin-12水平下降,下降程度与冠状动脉病变程度相关。

Level of Apelin-12 and correlation study in patients with coronary heart disease complicating diabetes ;mellitus

Objective To analyze the level of Apelin-12 and correlation study in patients with coronary heart disease （CHD） in different lesion degrees complicating diabetes mellitus （DM）. Methods The hospitalized patients （n=195, male 107 and female 88, aged from 34 to 87） examined with coronary angiography （CAG） were continuously chosen from Feb. 2011 to Feb. 2013, and then divided into 3 groups according to outcomes of CAG and complicating DM including normal group （n=59）, CHD group （n=75） and CHD complicating diabetes group （CHD＋DM group, n=61）. According to the outcomes of CAG, the patients were divided into control group （n=53）, 1-vessel group （n=58）, 2-vessel group （n=36） and multi-vessel group （n=48）. The outcomes of CAG were given Gensini scoring and level of serum Apelin-12 was detected. The correlation between Apelin-12 and CHD lesion degrees was analyzed. Results The level of serum Apelin-12 showed a decreasing trend in control group, 1-vessel group, 2-vessel group and multi-vessel group （4.35±1.27） ng/mL vs. （3.36±0.84） ng/mL vs. （2.28±0.33） ng/mL vs. （1.69 ±0.48） ng/mL], and Gensini integral showed a increasing trend in different groups （10.63±4.16） vs. （25.64± 8.53） vs. （47.18±16.32） vs. （65.18±18.43）, all P〈0.05]. The correlation analysis showed that Gensini integral was negatively correlated to the level of Apelin-12. Conclusion The level of Apelin-12 decreases in CHD patients, and its descending extent is correlated to coronary lesion degree.
