低剂量多巴酚丁胺负荷超声心动图结合DTI技术对存活心肌的评价

摘    要:目的:探讨低剂量多巴酚丁胺负荷超声心动图(LDDSE)结合多普勒组织成像技术(DTI)评价存活心肌的可行性。方法:12例急性心肌梗死患者于发病后进行LDDSE试验,选取静息时低动力或无动力心肌节段进行存活性评价,留取脉冲组织多普勒(PW-DTI)频谱图。结果:根据随访时室壁运动的改善情况,将27个受检节段分为存活组(A组)与非存活组(B组)。试验中,A组与B组的收缩期峰速度(Vs)均随剂量的增加有增大的趋势,且A组在各剂量阶段及随访时的Vs均大于B组。终止用药时的Vs与静息时相比的增量,A组大于B组。余指标未见规律性变化。结论:LDDSE结合DTI应用于存活心肌的检测是可行的,Vs是主要的观测指标。

Abstract:Objective: To evaluate the feasibility of identifying viable myocardium by applying low dose dobutamine echocardiography (LDDSE) combined with Doppler tissue imaging (DTI). Methods: 12 patients with acute myocardial infarction were enrolled and each of them took LDDSE test after infarction. The hypokinetic or akinetic segments were employed to identify the viability. Pulsed-wave Doppler tissue imaging (PW-DTI) of each segment selected was retained to further analysis. Results: 27 selected segments were divided into two groups which were viable myocardium (groupA) and non-viable myocardium (group B) depending on the improvement of wall motion during follow-up. During LDDSE both groups showed the tendency of increased peak velocity of contraction phrase (Vs). However, the Vs of group A was larger than that of group B at each dose during the follow-up. The increase of Vs from rest to end-point in group A was larger than that of group B. The other parameters didn't show regular change during LDDSE. Conclusions: It is feasible to identify the viability of myocardium by LDDSE combined with PW-DTI.
