冠心病痰热证患者心率变异性分析

目的 通过对痰热证冠心病患者的心率变异性(HRV)进行分析,探讨此证型患者的心脏自主神经功能改变特点及临床意义.方法收集经冠状动脉造影或明确心肌梗死病史等确诊为冠心病的患者79例,辨证为痰热证组30例,血瘀证组30例,2组均进行动态心电图检查,分析HRV时域参数.结果 痰热证组HRV时域参数明显低于血瘀证组(P＜0.01).痰热证组HRV参数SDNN在不同降低程度(SDNN≤50 ms、100 ms＞SDNN＞50 ms)的发生率明显高于血瘀证组(P＜0.01).结论 冠心病痰热证患者HRV时域指标降低明显,提示自主神经损害严重,预示心血管事件中猝死风险增加.

Analysis of Heart Rate Variability of Coronary Heart Disease Patients with Phlegm Heat Syndrome

Objective To discuss the characteristics and clinical significance of cardiovascular autonomic nervous function changes through analysis of heart rate variability（HRV） of patients with coronary heart disease（CHD） and phlegm heat syndrome.Methods Seventy-nine patients were diagnosed as CHD by coronary angiography or myocardial infarction medical history.A group of 30 cases with phlegm heat syndrome and another group of 30 cases with blood stasis syndrome were selected from them.Then the time domain parameters of HRV in Holter testing of the 2 groups were compared.Results The time domain parameters of phlegm heat syndrome group were less than that of blood stasis syndrome group obviously（P0.01）.The incidence rate of SDNN of phlegm heat syndrome group in different decrease degree（SDNN≤50 ms,100 msSDNN50 ms） was higher than that of the blood stasis syndrome group obviously（P0.01）.Conclusion For patients with CHD with phlegm heat syndrome,obviously decreasing of HRV time domain parameters suggests serious damage of autonomic nervous,and indicates high risk of sudden death in cardiovascular events.
