食管与贲门腺癌的临床病理之特点

目的比较分析食管腺癌与贲门癌的临床和病理特点，探讨两者的异同点。方法回顾、对比分析本院4年间收治的48例原发性食管腺癌，322例贲门癌的年龄和性别分布、主要症状、病理类型及分期、淋巴结转移情况、早期诊断率和手术切除率并进行统计学分析。结果食管腺癌、贲门癌多好发于老年男性。除食管腺癌的反酸及烧心症状的发生率（52．1％）高于贲门（24．2％）外，其他症状发生率两者相近。食管腺癌、贲门癌早期诊断率分别是8．3％和16．1％。食管腺癌、贲门癌手术切除率分别为81．3％和77％，根治性切除率为70．8％和68．3％。结论食管腺癌与贲门癌在一般临床特征上相似，但淋巴结播散类型不同。食管腺癌与贲门癌早期诊断率低，但手术切除率仍然较高，对其手术治疗应持积极态度。

The clinical and pathological features of es0phageal adenocarcinoma and cardia adenocarcinoma

Objective To discuss the difference between esophageal adenocarcinoma (EA) and cardia adenocarcinoma(CA) by contrasting the clinical and pathological features of them. Methods The distribution of age and sex,chief complaint,pathological category and staging ,lymph node metastasis,the early diagnostic rate and the rate of resection of 48 cases of EA and 322 cases of CA treated in our hospital in the past 4 years were analysed respectively.SPSS software was used to analyse the data. Results There were high incidences of EA and CA in senilc male. Except that the frequency of acid reflux and heartburn(52.1%) were higher than CA(24.2%), the frequencies of EA's other symptoms were similar to those of CA. The early diagnostic rate of EA and CA were 8.3%,16.1% respectively. The rate of resection of EA and CA were 81.3%,77% respectively. The rate of radical resection of EA and CA were 70.8%,68.3% respectively. Conclusions The clinical features of EA are similar to those of CA, but the types of lymph node metastasis between them are different. Although the early diagnostic rate of EA and CA are low, the rate of resection are still relative high. So we should have a positive attitude to surgical therapy.
