紫癜性肾炎患儿血清基质金属蛋白酶水平与肾脏损害的关系

摘    要:目的探讨基质金属蛋白酶-9(MMP-9)、基质金属蛋白酶抑制因子-1(TIMP-1)和肿瘤坏死因子-α(TNF-α)在紫癜性肾炎(HSPN)患儿中的作用及其与肾脏损害的关系。方法收集24例紫癜性肾炎(HSPN)、27例过敏性紫癜(HSP)患儿及20例健康对照组儿童的血清。采用酶联免疫吸附法(ELISA)检测各组儿童血清MMP-9、TIMP-1及TNF-α的含量,生化分析仪检测HSPN组和HSP组儿童的血清肌酐(Cr)、β2-微球蛋白(β2-MG),双缩脉法检测24 h尿蛋白定量。结果①HSP组和HSPN组患儿的血清MMP-9、TIMP-1及TNF-α水平均高于对照组。②HSPN组患儿血清β2-MG、24 h尿蛋白量较HSP组患儿显著增高;HSP组患儿血清β2-MG和血Cr高于对照组,24 h尿蛋白无显著性差异。③HSPN组患儿血清MMP-9及TNF-α含量与24 h尿蛋白定量、血β2-MG、Cr呈显著正相关;血清TIMP-1含量与24 h尿蛋白定量、血β2-MG、血Cr、均无显著相关性。结论血清MMP-9水平增高与肾脏损害的程度密切相关,血清MMP-9可能参与了HSPN的发生发展。

Abstract:Objective To explore the role and the relationship of matrix metalloproteinases-9(MMP-9),tissue inhibitor of metalloproteinase-1(TIMP-1),tumor necrosis factor-α on children with Henoch-Schonlein purpura nephritis. Methods Serum of 24 children with Henoch-Schonlein purpura nephritis(HSPN),27 children with Henoch-Schonlein purpura(HSP)as well as 20 healthy children.Enzyme-linked immunosorbent(ELISA) was adopted to detect the content of MMP-9,TIMP-1and TNF-α in those children′s serum.Biochemical analyzer was also applied to check the content of serum creatinine(Cr),c(β2-MG)in the HSPN group,HSP group.Meanwhile,twenty-four hours urinary protein content was checked by means of biuret. Results ①The serum level of MMP-9、TIMP-1and TNF-α in group HSP and HSPN was much higher than that of controlled group.②The serum of β2-MG and twenty-four hours urinary protein content in group HSPN was obviously higher than that of group HSP;While the serum of β2-MG and Cr in group HSP was higher than the controlled group when there was no significant difference of twenty-four hours urinary protein content.③There were significantly positive correlations between the serum content of MMP-9 as well as TNF-α and the twenty-four hours urinary protein content,β2-MG and Cr;Yet there was no correlation between the serum content of TIMP-1,twenty-four hours urinary protein content,β2-MG and Cr. Conclusion The increase of serum MMP-9 is closely related to the kidney damage in which serum MMP-9 is probably involved in the development of HSPN.
