单纯神经麻风：聚合酶链反应的诊断价值

通常，若检测不到抗酸杆菌（AFB），则难以诊断单纯神经麻风（PNL）。本研究旨在对聚合酶链反应（PCR）在诊断PNL中的作用进行评估，对58例（41例男性和17例女性）疑似PNL患者进行检测，将患者分为界线类偏结核样型麻风（BT，40例）和结核样型麻风（TT，18例）。对患者实施神经活检，20例患者（均为BT患者，占患者总数的34．5％）的AFB检查为阳性，从患者的神经活检样本中提取DNA，使用PCR对麻风分枝杆菌DNA的特殊重复序列进行扩增。29例（50％）患者神经活检样本的PCR分析为阳性，其中27例为BT型，2例为TT型患者。而且，神经活检显示AFB为阴性的38例患者中14例的PCR分析为阳性，其中12例为BT型，2例为TT型患者。研究证实PCR分析可作为探讨PNL的有效方法，对于神经活检显示AFB为阴性的患者，使用PCR分析可对超过1／3的患者作出确诊。

Patients with chronic wounds relevant contact sensitization. frequently acquire clinically We examined the results of the skin patch tests in patients with chronic wounds, who were treated between 1999-2004 in the Department of Dermatology, University School of Medicine, Essen, Germany. Altogether 105 patients with chronic wounds underwent patch testing. In 68 individuals, contact sensitization to at least one substance was detected. The most frequent contact allergens were to balsam of Peru, Amerchol L - 101, fragrance mix, wool wax alcohols and rosins. Contact sensitization to wound dressing materials was also identified in 4 patients. Our results demonstrate the current spectrum of contact sensitization of patients with chronic wounds. Our results show the importance of paying attention to the ingredients in wound dressings and performing patch testing if there is any clinical suggestion of allergic contact dermatitis.
