厄贝沙坦和氨氯地平治疗老年高血压病合并阵发性房颤的临床疗效对比观察

目的 对比观察厄贝沙坦和氨氯地平治疗老年高血压病合并阵发性房颤的临床疗效.方法 选择2011年3月至2013年3月我院接诊的70例老年高血压病合并阵发性房颤患者进行研究.按照随机数表法分为两组,分别采用厄贝沙坦和氨氯地平对患者进行治疗.治疗后,观察两组患者治疗前后血压变化情况、心功能状况及房颤发生情况.结果 治疗后,厄贝沙坦组和氨氯地平组患者的收缩压、舒张压均明显下降（P＜0.01）.两组患者的左心功能状况均出现明显改善（P＜0.01）,厄贝沙坦组患者的左房舒张末期前后径（LADd）、左室重量指数（LVMI）值明显低于氨氯地平组患者（P＜0.01）,厄贝沙坦组患者的房颤持续时间明显短于氨氯地平组（P＜0.05）,房颤发作频率及房颤发生率明显低于氨氯地平组（P＜0.05）,房颤第一次发作间隙明显长于氨氯地平组（P＜0.05）.结论 厄贝沙坦治疗老年高血压病合并阵发性房颤效果良好,可有效降低患者血压,改善患者的房颤发生状况,值得临床推广应用.

Clinical observation of irbesartan and amlodipine treatment for elderly patients with paroxysmal atrial fibrillation hypertension merger

Objective To compare the clinical efficacy of irbesartan and amlodipine treatment for elderly patients with paroxysmal atrial fibrillation Hypertension merger. Methods 70 cases of elderly patients with hypertension combined with paroxysmal atrial fibrillation from March 2011 to March 2013 in our hospital were studied . According to a random number table divided into two groups, respectively irbesartan and amlodipine treatment of patients. After treatment, the changes of blood pressure, cardiac function and atrial fibrillation patients before and after treatment were recorded. Results After treatment, irbesartan and amlodipine groups of patients with systolic blood pressure, diastolic blood pressure decreased significantly（P 〈 0. 01 ）. Left ventricular function in patients with both conditions occur both groups improved significantly （P 〈 0. 01 ）, irbesartan group of patients with diastolic left atrial anteroposterior diameter （LADd）, left ventricular mass index （LVMI） was significantly lower than in the amlodipine group patients （ P 〈 0. 01 ）, the duration of atrial fibrillation of irbesartan group was significantly shorter than the amlodipine group （ P 〈 0. 05 ） , the frequency of atrial fibrillation and atrial fibrillation was significantly lower in the amlodipine group （P 〈 0. 05 ）, the first episode of atrial fibrillation was significantly longer than the gap amlodipine group （P 〈 0. 05 ）. Conclusions Irbesartan treatment for elderly hypertensive patients with paroxysmal atrial fibrillation has good effect, can effectively reduce blood pressure, improve the patientg atrial fibrillation condition, worthy of clinical application.
