浅表性膀胱肿瘤二次电切的效果观察

目的 探讨浅表性膀胱肿瘤采用经尿道二次电切术的临床效果.方法 将60例经尿道膀胱肿瘤电切术治疗的浅表性膀胱肿瘤患者随机分为两组.对照组于首次手术后采用丝裂霉素膀胱灌注化疗;观察组在首次电切治疗4周后进行二次电切治疗,并进行丝裂霉素膀胱灌注治疗,比较两组的效果.结果 所有患者均随访6个月以上,平均（13.5±4.6）个月,对照组总体复发率60.0％,观察组总体复发率20.0％,差异有统计学意义（P＜0.05）;对照组高级别肿瘤的复发率为68.2％ （15/22）,观察组高级别肿瘤的复发率为21.7％ （5/23）,差异有统计学意义（P＜0.05）.结论 经尿道二次膀胱肿瘤电切术可明显降低浅表性膀胱肿瘤的术后复发率,更准确地进行肿瘤分期和发现残存肿瘤,值得临床推广应用.

Observation on effect of secondary Trans urethral resection of superficial bladder tumor

Objective To observe the clinical effects of secondary Trans urethral resection of bladder turnout （TURBt） on superficial bladder tumor. Methods 60 cases of transurethral resection of bladder tumor patients were selected and divided into two groups. In control group, 30 patients were treated by TURBt and intravesical mitomycin chemotherapy only after operation. In observation group, 30 patients were treated by TURBt and by secondary TURBt 4 weeks after first operation. Patients were also treated by intravesical mitomycin chemotherapy after operation. The effects of the two groups were compared. Results All patients were followed for six months or more, average （13. $ ＋4.6） months. The total recurrence rate of observation group was lower than control group （ P 〈 0. 05 ）. The recurrence rate of high-grade tumour of observation group was also lower than control group （P 〈 0. 05）. Conclusions Trans urethral resection of bladder tumour can significantly reduce the recurrence rate of superficial bladder tumors and can staging tumors, found residual tumors more accurately. It is worth for generalization.
