探讨无创正压通气在急性左心衰竭中的应用价值

目的研究无创正压通气对急性左心衰竭患者的治疗效果。方法49例急性左心衰竭患者随机分为2组，Bi—PAP组23例在应用常规抗心衰药物的同时联用BiPAP呼吸机进行无创正压通气治疗；对照组26例只应用常规抗心衰药物治疗，记录治疗前后症状、体征、及血气分析各参数。结果BiPAP组患者临床症状、体征、血气等明显改善，显效率73．91％，总有效率95．23％；对照组显效率26．92％，总有效率69．23％，两组间显效率差异有统计学意义。结论无创正压通气治疗急性左心衰时能迅速改善患者症状和低氧血症，改善心肺功能，是一种快速安全有效的抢救方法。

Study on therapeutic effects of non invasive positive pressure ventilation in acute left heart failure

Objective To observe the therapeutic effects of acute left heart failure with non invasive positive pressure ventilation（NIPPV）. Methods Randomly dividing 49 cases into 2 groups, BiPAP group with 23 cases was treated with BiPAP respiratory machine for non invasive positive pressure ventilation therapy on the base of routine anti - heart failure medicine, the control one with 26 cases was treated with anti heart failure drugs, their symptoms, signs and blood - gas analysis were record, then all parameters were analyse. Result In BiPAP group, the clinical symptoms, signs and blood - gas were much improved, the marked effective rate 73.91% , the total effective rate 95.23% ; 26. 92% and 69.23% for the control one respectively; two groups had significant difference. Conclusion NIPPV for treating acute left heart failure can speedily relieve patients ＇ symptoms and low oxygen blood syndrome, improve cardiac function and pulmonary function, which makes it a quick, safe and effective rescue.
