经皮腔内冠状动脉成形术对冠心病患者循环血内皮素水平的影响

来研究观察PTCA后股动脉血ET浓度的变化以探讨PTCA对ET释放的影响。14例行PTCA患者分别于术前、术后即刻及术后1小时取股动脉血,20例单纯行冠状动脉造影(CAG)的患者分别于术前及术后即刻取股动脉血,用放射免疫分析法测血浆ET浓度。结果:PTCA术后早期股动脉血ET水平明显升高(从69.87±31.74pg/ml到79.09±31.18pg/ml,P<0.05),术后1小时继续升高(83.65±39.69pg/ml)。术后血ET浓度升高值与球囊扩张后直径比被扩张动脉直径之比(B/A)呈正相关(R=0.589,P<0.05)。CAG术后血浆ET浓度较术前无变化(分别为59.36±22.57pg/ml和54.20±18.70pg/m1,P>0.05)。结论:PTCA使循环ET水平升高,内皮损伤可能是引起ET释放和合成的主要因素。

EFFECT OF PERCUTANEOUS TRANSLUMINAL.CORONARY ANGIOPLASTY ON CIRCULATING ENDOTHELIN LEVELS

For determining the effect of percutaneous transluminal coronary angioplasty (PTCA) on the circulating levels of endothelin (ET) . We measure the femoral artery plasma concentration of ET with radioimmunoassay in 14 patients who undergo PTCA before the procedure,immediately after the end of the procedure and 1 hour later. We also measure the femoral artery plasms concentration of ET with radioimmunoasssay in 20 patients who undergo coronary angiography(CAG) alone before and immediately after CAG. Results:There was a significantly increase in plasma ET concentration immediately after PTCA and 1 hour later(from 69. 87?1. 74pg/ml to 79. 09 ?34. 18pg/ml and 83. 65?6. 69pg/ml p<0. 05). The diameter of inflated balloon/the diameter of the dilated artery ratio(B/A ratio) correlated with plasma ET concentvration rise(r = 0. 589 p<0. 05). There was no change in the plas maETconcentrationafterCAGalone(599. 36?2. 57pg/mlVS. 54. 20?8. 70pg/mlP> 0. 05). Conclusions::PTCA influences the release of ET. Endothelial damage may relate with the increase of plasma ET levels.
