多层螺旋CT多平面重建对周围型小肺癌的诊断价值

摘    要:目的探讨多层螺旋CT多平面重建(MPR)对周围型小肺癌的诊断价值。方法收集了52例经病理证实的周围型小肺癌,对其多层螺旋CT的MPR和横断高分辨薄层图像作对比分析。结果MPR在肿瘤分叶征、细支气管气相、血管集束征、胸膜凹陷征、叶间胸膜破坏征的显示率明显高于横断位薄层。棘突征、短毛刺征的显示率与横断位薄层大致相同;空泡征的显示率反而较横断位薄层低。结论多层螺旋CT的MPR可以弥补横断位图像显示肿瘤征象不全面的缺点,对周围型小肺癌有重要的诊断价值。

Abstract:Objective To probe the value of multi-planar reconstruction (MPR) of multi-slice spiral CT in diagnosing peripheral small lung carcinoma. Methods The findings on the MPR images and high-resolution thin-slice axial images were compared in 52 patients with pathologically proved peripheral small lung carcinoma. Results The incidence of lobulation, air-bronchogram, vessel convergence, pleural indentation sign and break of interlobar pleura detected on MPR images was significantly higher than that on thin-slice axial images. The incidence of spine sign and short spiculation detected on MPR images was similar to that on thin-slice axial images. The vacuole sign was demonstrated better on thin-slice axial images than on MPR images. Conclusion MPR images of multi-slice spiral CT, complementing delineation of the tumoral findings, which incompletely demonstrated on thin-slice axial images, play an important role in diagnosing peripheral small lung carcinoma.
