眼反应分析仪在临床眼压测量中的应用

摘    要:目的 比较眼反应分析仪(ORA)与压平式(GAT)、非接触式眼压计(NCT)眼压测量的差异。方法 对正常组、青光眼组及屈光术后组(分别为168眼、62眼、102眼)行ORA测量,正常组及屈光术后组行非接触式眼压计测量,青光眼组行压平眼压计测量,各组均用超声角膜测厚仪测量中央角膜厚度(CCT)。结果 正常组、青光眼组及屈光术后组平均ORA测量值分别为(18.33±4.00) mmHg、(19.91±4.44) mmHg、(16.69±5.24) mmHg,NCT/GAT值分别为(17.44±3.53) mmHg、(18.90±3.74) mmHg、(13.91±4.99) mmHg),各组平均ORA测量值均高于NCT/GAT值;正常组及屈光术后组NCT值与CCT值存在中度相关性(r分别为0.466、0.472),青光眼组GAT值与CCT值存在中度相关性(r为0.434),ORA测量值在各组中与CCT均仅存在低度相关性(r分别为0.258、0.221、0.249)。结论 与NCT及GAT比较,ORA测量值较少受角膜因素影响,能提供更接近真实眼压的测量结果。

Abstract:Objective To compare ocular response analyzer(ORA), goldmann applanation tonometer(GAT)and non-contact tonometer (NCT)in the intraocular pressure measurement. Methods The patients were recruited as the normal group(168 eyes), the glaucoma group(62 eyes) and the post-refractive-surgery group (102 eyes). The intraocular pressure and the central corneal thickness(CCT) of all patients were measured by ORA and A-type ultra-sonography. The intraocular pressure of the normal group and the post-refractive-surgery group was measured by NCT and GAT in the glaucoma group. Results In the normal group and the post-refractive-surgery group, NCT value was (17.44±3.53) mmHg and (13.91±4.99) mmHg respectively. In the glaucoma group, GAT value was (18.90±3.74) mmHg. In all three groups, ORA values were(18.33±4.00) mmHg, ( 19.91±4.44) mmHg and (16.69±5.24) mmHg)respectively. ORA values were higher than NCT or GAT value in all groups. There were medium-level correlations between CCT value and NCT value in normal group and post-refractive-surgery group(r=0.466,0.472).There was a medium-level correlation between CCT values and GAT value in the glaucoma group (r=0.434).There was a low-level correlation between CCT value and ORA value in all groups(r=0.258, 0.221,0.249). Conclusion Compared with the goldmann applanation tonometer and non-contact tonometer, ORA can provide more accurate IOP value and be less influenced by corneal factors.
