帕瑞昔布超前镇痛对全凭静脉麻醉腹腔镜胆囊切除术后疼痛的影响

目的探讨帕瑞昔布超前镇痛对全凭静脉麻醉腹腔镜胆囊切除术后疼痛的影响。方法采用对照研究的方法,将56例行腹腔镜胆囊切除术的患者随机分为对照组和观察组各28例,观察组于麻醉前30min静脉注射帕瑞昔布,术毕静脉注射等体积生理盐水;对照组于麻醉前30min注射生理盐水,而术毕时静脉注射帕瑞昔布。记录所有患者术后24h阿片类镇痛药使用量,比较两组患者手术清醒后24h疼痛评分。结果观察组术后1、4、8、12及24h疼痛程度评分均明显低于同期对照组疼痛程度评分,差异均有统计学意义(P<0.05);术后24h内,对照组患者各观察时点阿片类药物累积用量均高于观察组,且术后1、4、8及12h,两组比较差异具有统计学意义(P<0.05)。结论帕瑞昔布超前镇痛对于腹腔镜胆囊切除术患者术后疼痛的干预效果良好,能够显著减少术后阿片类药物的使用量。

The effect of pre-operative parecoxib injected on postoperativepain after laparoscopic cholecystectomy of total intravenous anesthesia

Objective To explore the effect of pre-operative parecoxib injected aspreemptive analgesia on postoperative pain after laparoscopic cholecystectomy of total intravenous anesthesia. Methods 60 patients who underwent elective laparoscopic cholecystectomy under general anesthesia were randomly divided into twogroups, the control group and the observation group.Patients of the observation groups were given parecoxib infusion 30 min before anesthesia and normal saline infusion atthe time of suturing skin, while patients of the control group were given normal saline infusion 30 min before anesthesia and 40 mg parecoxib infusion at the time of suturing skin. The consumption of analgesics was recorded, and the degree of the postoperative pain was assessed in the first 24 h after surgery.Results Analgesic consumption was significant difference among the two groups. Compared with the control group,a nalgesic consumption in observation group was obviously reduced with statistical significance（P0.05）.Conclusion Parecoxib preemptive analgesia is good effcacy for laparoscopic cholecystectomy patients with postoperative pain, which can significantly reduce the amount of postoperative opioid use.
