摘    要:牙周炎导致的牙列缺失常伴随严重的牙槽骨吸收,甚至出现上下颌骨体的吸收。松动牙的拔除时机和标准,软硬组织量不足,不同区段骨吸收量不均一,咬合关系异常或不稳定,永久修复前的缺牙期,种植义齿的固位方式、被动就位、咬合设计,防止修复后的种植体周围骨吸收,维持口腔自洁作用及降低患者远期维护成本等问题均是全颌固定种植义齿修复设计时需要考虑的关键因素。本文总结分析四川大学华西口腔医学院种植中心分段式全颌固定种植义齿修复病例,探讨修复设计、种植手术、早期过渡义齿修复和永久修复过程中遇到的问题及思考。

Abstract:Periodentitis is one of major risk factors for edentulous and induces dramatic jaw bone absorption. When designing segmented fixed implant supported denture we should consider the following key factors: timing and principles of these loose teeth extraction, defects of hard and soft tissue, the diversity of bone absorption in different area, disorders and instability of maxillomandibular occlusion, edentulous before final prosthodontic, retentive patterns of implant supported denture, occlusion designs, preventing bone absorption surrounding implants, maintaining oral health and reducing long-term maintenance costs. This article summarizes and analyzes the patients with segmented fixed implant supported denture in the Implant Center of West China College of Stomatology, discusses the problems and the solutions during the process of restoration design, implant surgery, and immediate and final restoration.
