生物可降解吻合环用于肠吻合术的安全性评价的Meta分析

摘    要:目的客观评价生物可降解吻合环(BAR)用于肠吻合术的有效性及安全性。方法计算机检索1989年1月至2010年7月发表的比较BAR与手工吻合用于肠吻合术的随机对照实验。对所检索的文献进行jadad评分,评分≥3分的文献入选。用Revman 4.2软件进行统计分析。结果共纳入3篇随机对照试验文献,共651例患者。Meta分析结果显示,BAR用于肠吻合术与手工缝合相比在安全性上差异无统计学意义(P〉0.05),术后切口感染率比较OR=1.02,95%CI(0.50,2.07)]、术后吻合口瘘发生率比较OR=1.07,95%CI(0.39,2.91)]、术后肠梗阻发生率比较OR=2.37,95%CI(0.84,6.68)]、术后吻合口出血发生率比较OR=0.94,95%CI(0.34,2.55)],差异均无统计学意义(P〉0.05)。结论生物可降解吻合环用于肠吻合术安全可行。

Abstract:Objective To evaluate the availability and safety of biofragmentable anastomosis ring(BAR)in intestinal anastomosis.Methods A computer-based search was conducted By meta-analysis we made a comprehensive analysis of the risk factors for BAR and manual repairs in intestinal anastomosis based on 19 articles published in China between January and July 2010 were retrieved;The quality of included trials was evaluated by Jarad system,literature≥3 points were selected.Revman 4.2.8 software was used for statistical analysis.Results Three randomized controlled trials(651 patients)undergoing BAR versus suture were included.The results of meta-analysis indicated that statistically significant differences had not been observed between the two operation:postoperative wound infectionOR=1.02,95%CI(0.50,2.07)];postoperative anastomotic leakageOR=1.07,95%CI(0.39,2.91)];postoperative intestinal obstructionOR=2.37,95%CI(0.84,6.68)];postoperative anastomotic bleedingOR=0.94,95%CI(0.34,2.55)].Conclusion BAR could be safely applied in intestinal anatomosis.
