奎硫平与氯氮平对首发精神分裂症患者生活质量的影响

目的探讨奎硫平、氯氮平治疗对首发精神分裂症患者生活质量的影响。方法将60例精神分裂症患者随机分为奎硫平及氯氮平治疗组各30例进行对比治疗。治疗前、出院时及出院3mo末采用阳性与阴性症状量表、生活质量综合评定问卷、副反应量表评定临床疗效、生活质量及不良反应。结果两组出院时及出院3mo末阳性与阴性症状量表评分均显著低于治疗前（P〈0.01），同期两组比较均无显著性差异（P〉0.05）；出院3mo末生活质量综合评定问卷评定奎硫平组躯体功能、心理功能、社会功能及总体生活质量得分均显著高于氯氮平组（P〈0.05或0．01），副反应量表评定奎硫平组显著低于氯氮平组（P〈0．01）。结论奎硫平、氯氮平治疗精神分裂症疗效相当，均可改善患者的生活质量，但奎硫平治疗不良反应发生率较低，改善生活质量优于氯氮平。

Effects of quetiapine vs. clozapine on the quality of life in first-episode schizophrenics

Objective To compare the effects of quetiapine and clozapine on quality of life in first-episode schizophrenics. Methods Sixty schizophrenics were randomly divided into quetiapine group(n=30) and clozapine group(n=30).The curative effects and adverse reactions were assessed with the Positive and Negative Symptom Scale(PANSS),the General Quality of Life Inventory (GQOLI)and the Treatment Emergent Symptom Scale(TESS) before treatment,at discharge and at the end of 3rd month after discharge,respectively. Results At discharge and at the end of 3rd month after discharge scores of the PANSS were significantly lower compared with pre-treatment for both groups(P<0.01),but there was no significant difference between the 2 groups(P>0.05);at the end of 3rd month after discharge,scores of physical,mental and social function and total score of the GQOLI were significantly higher in the quetiapine group than in the clozapine group(P<0.05 or 0.01); the scores of TESS were significantly lower in quetiapine group than in clozapine group(P<0.01). Conclusion Quetiapine and clozapine have equivalent curative effects and both could improve the quality of life for schizophrenics,but incidence of adverse effects of quetiapine was lower and it is superior to clozapine in improving quality of life.
