肠内营养乳剂在胃癌患者术前肠道准备中的应用

目的 探讨肠内营养乳剂辅助饮食管理法在胃癌患者术前肠道准备中的应用.方法 选择2012年6-12月入住上海交通大学医学院附属新华医院的胃癌患者80例,随机分为实验组和对照组各40例.术前肠道准备中,实验组采用肠内营养乳剂(瑞代),对照组采用聚乙二醇,评估两组患者术中肠道清洁程度；观察患者肠道准备期间不良反应与术后肠道功能恢复情况.结果 两组患者术中肠道清洁程度比较,差异无统计学意义(P＞0.05)；实验组肠道准备期间出现腹胀、恶心和乏力等不良反应的发生率低于对照组；实验组术后肠道功能恢复较早,两组比较差异有统计学意义(P＜0.05).结论 瑞代用于肠道准备能达到良好的肠道清洁要求,且不良反应少,肠道功能恢复时间早,有助于手术患者的早日康复.

Application of enteral nutrition emulsion - assisted preoperative bowel preparation in patients with gastric cancer

Objective To investigate the application of enteral nutrition emulsion-assisted preoperative bowel preparation in patients with gastric cancer. Methods A total of 80 patients undergoing gastric operation in Xinhua Hospital, Shanghai Jiao Tong University School of Medicine were randomly and equally divided into the polyethylene glycol （PEG） group and enteral nutrition emulsion group from June to December 2012. The PEG group received PEG for bowel preparation, while the other group received oral enteral nutrition emulsion 1 000 mL/d for 3 d for bowel preparation. The degree of cleanliness during colonoscopy, and the adverse events including abdominal distention, abdominal pain, nausea and fatigue between the two groups were compared. The post- operative time to first flatus and defecation was observed. Results There was no significant difference in colon cleansing between the two groups（ P 〉 0.05 ）. While the incidences of adverse events and adverse reactions were significantly lower in enteral nutrition emulsion group （ P 〈 0.05 ）, and the postoperative time to first flatus and defecation was shorter in enteral nutrition emulsion group （P 〈 0. 05）. Conclusion Enteral nutrition emulsion can be applied in preoperative bowel preparation with liquid diet. It can acquire the satisfying colon cleanliness, reduce the adverse reactions, and promote postoperative rehabilitation in the patients with gastric cancer.
