不同脱机方法对机械通气患者脱机效果的影响

目的 探讨不同脱机方法对机械通气患者脱机效果的影响.方法 对60例机械通气患者进行回顾性对比研究,以2005年1月-2006年8月20例患者作为对照组,2006年9月-2007年10月40例患者作为观察组,对照组采用传统脱机方法,观察组在传统脱机方法基础上,实行机器空转以减轻患者心理顾虑,并建立撤机日志.比较两组患者致残率、脱机成功所需天数及脱机耐受程度.结果 观察组脱机成功时间≤5 d的例数33例(83%),对照组脱机成功时间≤5 d的例数9例(45%)(P<0.05);观察组脱机能耐受32例(80%),对照组脱机能耐受11例(55%)(P<0.05);按照格拉斯哥预后评分标准,观察组患者预后明显优于对照组(P<0.01).结论 脱机过程中实行机器空转并建立撤机日志能明显提高患者的脱机成功率和脱机耐受程度.
