摘    要:目的探讨椎基底动脉迂曲扩张症(VBD)的MRI、MRA表现及其临床应用价值。方法回顾分析19例VBD的影像资料,分析MRA显示的异常血管形态及MRI显示的脑组织信号、颅神经走行改变,总结其临床特点和影像特征。结果19例患者MRA表现为椎基底动脉迂曲、扩张,直径为5~20mm,长度约为31~35mm;其中10例向右侧偏曲,9例向左侧偏曲;5例并大脑后动脉狭窄;9例血管与颅神经关系密切,7例MRI平扫见后循环梗死灶。结论VBD的临床表现多样,常规MRI联合MRA可清楚显示血管解剖及其与后颅窝脑组织关系,有助于VBD的确诊及全面综合评估。

Abstract:Objective To investigate the manifestations of MRI and MRA of ertebrobasilar dolichoectasia (VBD) and their clinical value. Methods We retrospectively analyzed the imaging data of 19 patients with VBD, the abnormal vascu- lar morphology shown by MRA as well as and the direction and the signal changes of brain tissue and cranial nerve shown by MRI and, also summarized their clinical features and imaging characteristics. Results In these cases, MRA showed that vertebral basilar artery was tortuosity and dilatation, with 5~20 mm in diameter and about 31~35 mm in length. Of these 19 patients, 10 presented with right deviation and 9 with left deviation, 5 combined with stenosis of posterior cere- bral artery, 9 were closely related to blood vessels and cranial nerves, and 7 were observed posterior circulation infarcts by MRI scan. Conclusion The manifestations of VBD are varied in clinic. The conventional MRI combined with MRA can clearly display vascular anatomy and its relationship with posterior fossa brain tissue, which could help the diagnosis and comprehensive evaluation of this disorder.
