腰硬联合麻醉前预充对剖宫产患者血压及恶心呕吐、寒颤发生率的影响

目的: 观察预充乳酸林格氏液对腰硬联合麻醉（CSEA）下剖宫产术中患者血压及呕吐、房颤发生率的影响。方法: 60例拟在CSEA下行剖宫产产妇，随机分为两组，实验组(R 组，n=30)于麻醉前30 min内输完乳酸林格氏液500ml，然后再以5ml/kg&#8226;h继续输注乳酸林格氏液，对照组(C 组，n=30)则直接以5ml/kg&#8226;h静脉输注乳酸林格氏液，观察两组患者麻醉前、麻醉后5min、10 min、20min、手术结束时的收缩压（SBP）、舒张压(DBP)变化和发生恶心呕吐及寒颤的情况。结果: 两组产妇麻醉前血压无差别，麻醉后5min、10min时实验组血压明显高于对照组（P<0.05），20min后两组血压无显著差异；实验组恶心呕吐发生率明显低于对照组（P<0.05）；两组寒颤发生率无明显差别。结论：麻醉前预充可以使剖宫产产妇术中血压相对稳定，降低恶心呕吐的发生率。

The Effects of Sodium Lactate Ringers Solution Expanding Blood Volume before combined Spinal-Epidural Anesthesia on Perioperative Blood Pressure, nausea-vomiting and shivering in cesarean section

Objective To observe the effects of sodium lactate Ringers Solution Expanding Blood Volume before combined Spinal-Epidural Anesthesia on Perioperative Blood Pressure, nauser-vomiting and shivering in cesarean section. Methods Sixty parturiens with term pregnancy(ASA Ⅰ～Ⅱ) receiving cesarean section were randomly divided into group R(experiment,n=30) and group C(control,n=30) , Group R was venously given 500ml of sodium lactate Ringers Solution 30 minutes before anesthesia , then 5mL&#8226;kg- 1&#8226;h - 1 after anesthesia. Group C was venously given 5mL&#8226;kg- 1&#8226;h - 1 of sodium lactate Ringers Solution immediately after be carried into operative room.. The changes of Perioperative SBP,DBP and and the incidence of nauser-vomiting and shivering in two groups were observed. Result SBP and DBP in group R were significantly increased than in group C on 5 and 10 min after the administration of anesthetics,but no statistical difference was observed after 20min after the administration of anesthetics between two group.The incidence of nausea-vomiting was decreased in group R than in group C. But the incidence of shivering was not different in two groups. Conclusion 　Preoperative transfusion of sodium lactate Ringers Solution was effective on stabilizing blood pressure. and preventing the incidence of nauser-vomiting.
