MMP-9、TIMP-1在膀胱癌中的表达及其与浸润转移的关系研究

摘    要:目的研究MMP-9和TIMP—1在膀胱癌中的表达情况,探讨其表达失衡与膀胱癌转移和预后的关系。方法应用S—P免疫组化法,检测86例膀胱癌MMP-9和TIMP—1表达情况,并行回顾性随访。结果在68例膀胱癌组中,MMP-9、TIMP-2阳性表达率分别为63.95%、59.30%。MMP-9阳性表达率随着肿瘤病理分级、浸润深度、淋巴结转移、TNM分期的升高而增加,而TIMP—1阳性表达率则呈下降趋势(P〈0.05)。MMP-9表达超过TIMP—1者膀胱癌发生肌层浸润、淋巴结转移的比例明显高于TIMP—1表达超过MMP-9者或二者表达平衡者,而MMP-9表达超过TIMP—1者生存率明显低于TIMP-1表达超过MMP-9者(P〈0.05)。结论MMP-9以促进肿瘤浸润转移为主,TIMP—1对膀胱癌有独立的抑制作用,MMP-9阳性表达而TIMP—1阴性表达者则其膀胱癌预后不良。MMP-9、TIMP—1表达可作为判断膀胱癌生物学行为和预后的重要指标。

Abstract:Objectives To investigate the expressions of matrix metalloproteinase-9 (MMP-9) and tissue inhibitor of metalloproteinase-1 (TIMP-1), and their correlation with the metastasis and prognosis in transitional cell carcinoma of bladder (TCCB).Methods The expression of MMP-9 and TIMP-1 were detected in 86 cases of TCCB by S-P immunohistochemistry, and their relationship with clinicopathological data was analyzed retrospectively.Results MMP-9 and TIMP-1 expressions were detected in 63.95% and 59.30% of TCCB cases respectively. The expression of MMP-9 increased along with higher pathological stage, infiltration depth, more lymph node metastases and higher TNM stages; however, the expression of TIMP-1 tended to decrease(p<0.05). Patients with the expression of MMP-9 over TIMP-1 had significantly higher tumor invasion, more lymph node metastases and lower survival rates after operation than those with the expression of TIMP-1 over MMP-9(p<0.05). Conclusions MMP-9 may promote tumor metastasis, while TIMP-1 inhibit. Intense MMP-9 expression and weak TIMP-1 expression indicate poor prognosis in TCCB. Expressions of MMP-9 and TIMP-1 may be considered as an important indicator for biological behavior and prognosis of bladder cancer.
