CXCR5~+CD8~+ T细胞在HIV感染中的免疫特征及其与疾病进展关系的研究

摘    要:目的探讨HIV感染者外周血CXCR5~+CD8~+T细胞的频率、功能变化及其与病情进展的相关性。方法收集40例HIV感染者和15例健康对照者,采用流式细胞分析术检测其外周血CXCR5~+CD8~+T细胞频率及IFN-γ和IL-10表达,并分析其与血浆HIV载量和外周血CD4~+T细胞计数的相关性。结果与健康对照组相比,HIV感染组外周血CXCR5~+CD8~+T细胞频率上调(P0.05),且与外周血CD4~+T细胞计数呈弱正相关(r=0.349,P=0.027),与血浆HIV载量呈弱负相关(r=-0.377,P=0.040);HIV感染者外周血CXCR5~+CD8~+T细胞IFN-γ表达与血浆HIV载量呈负相关(r=-0.514,P=0.002);而CXCR5~+CD8~+T细胞IL-10的表达在HIV感染者中明显上调(P0.05),与外周血CD4~+T细胞计数呈弱负相关(r=-0.317,P=0.046),与血浆HIV载量呈正相关(r=0.670,P=0.002)。结论 HIV感染者外周血CXCR5~+CD8~+T细胞频率功能变化与疾病进展密切相关。

Abstract:Objective To investigate the changes of the frequency and function of CXCR5+CD8+T cell subsets in peripheral blood of HIV infected patients and their correlation with disease progression. Methods Forty HIV infected patients and 15 health controls were enrolled in this study. The frequency of CXCR5+CD8+T cells and the levels of IFN-γ and IL-10 expression in peripheral blood were examined by flow cytometry. In addition, the correlation of the frequency and function of CXCR5+CD8+T cells with peripheral CD4+T lymphocyte counts and plasma HIV load were further analyzed. Results The frequency of CXCR5+CD8+T cells increased significantly in patients with HIV infection compared with those of health controls (P<0.05). In addition, the frequency of this subset in peripheral blood was slightly positively correlated with CD4+T cell counts (r=0.349, P=0.027), and was slightly negatively correlated with plasma viral load (r=-0.377, P=0.040). The IFN-γ expression level was also negatively correlated with plasma viral load in patients with HIV infection (r=-0.514, P=0.002). While IL-10 expression level was upregulated significantly in CXCR5+CD8+T cells (P<0.05) and it was slightly negatively correlated with CD4+T cell counts (r=-0.317, P=0.046) and positively correlated with plasma viral load (r=0.670, P=0.002) in HIV infected patients. Conclusions The changes of the frequency and function of CXCR5+CD8+T cells in peripheral blood are closely correlated with the progression of HIV infection.
