地佐辛联合舒芬太尼用于心脏手术后无线镇痛的效果

目的 评价地佐辛联合舒芬太尼用于心脏术后无线患者自控皮下镇痛（PCSA）的临床效果.方法 择期成人心脏手术患者80例,随机分两组,地佐辛联合舒芬太尼组（DS组）、舒芬太尼组（S组）,每组40例.DS组配方为地佐辛0.3 mg/kg与舒芬太尼1.5 μg/kg;S组为舒芬太尼3 μg/kg,两组均加盐酸昂丹司琼10 mg并用生理盐水稀释至156 ml.皮下给药速度2 ml/h.观察两组患者术后各点VAS评分、镇静程度评分（Ramsay）及72 h内PCSA有效按压次数、患者的不良反应及总体满意度.结果 两组VAS、Ramesy镇静评分及PCSA有效按压次数差异无统计学意义（P＞0.05）;DS组患者的总体满意度优于S组,且不良反应较少,差异有统计学意义（P＜0.05）.结论 地佐辛联合舒芬太尼用于心脏手术后PCSA无线镇痛效果确切可靠,患者满意度高且不良反应少.

Analgesic effects of dezocine and sufentanil with wireless system in patients after heart operation

Objective To evaluate the analgesic effects of patient-controlled subcutaneous an- algesia （PCSA） with dezocine and sufentanil by wireless system in patients after heart operation. Methods Eighty adult patients were scheduled for cardiac operation and randomly divided into dezo- cine with sufentanil group（group DS） and sufentanil group （group S）, 40 cases in each group. The formula of PCSA was dezocine 0.3 mg/kg and sufentanil 1.5 μg/kg in group DS, while sufentanil 3 μg/kg in group S and ondansetron 10 mg in each group, diluted to 156 ml with 0. 9% normal saline with the delivery speed at 2 ml/h. VAS, Ramsay score, effective pressing times of PCSA during 72 h, side-effects and patients＇ satisfaction were recorded. Results No differences were found in VAS, Ramsay scores and the effective pressing times between the two groups （P〉0.05）. Patients＇ satisfaction in group DS was significantly better than that in group S, and with fewer side-effects （P〈0.05）. Conclusion PCSA with dezocine and sufentanil by wireless system is an effective method for the cardiac postoperative analgesia, with higher patients＇ satisfaction and fewer side-effects.
