| 摘 要： | 目的促进替考拉宁的合理使用。方法分析1例57岁肾功能不全患者因"感染性休克(弥漫性腹膜炎)"予替考拉宁抗革兰阳性菌感染的治疗过程。结果肾功能不全患者使用替考拉宁需根据肌酐清除率调整剂量,且连续性血液净化与间歇血液透析时剂量调整略有不同。结论临床药师通过监护患者的肾功能与替考拉宁的剂量调整,可使临床用药更加安全、有效、合理。

| Abstract: | Objective To promote the rational use of teicoplanin. Methods A 57-year- old patient with renal insufficiency was ana- lyzed on the treatment process using teicoplanin for anti-Gram positive bacteria due to infectious shock（diffuse peritonitis）. Results In the patients with renal insufficiency, the dose of teieoplanin should be adjusted according to the creatinine clearance and the dose ad- justment in continuous renal replacement therapy or intermittent hemodialysis was slightly different. Conclusion By monitoring the renal function of the patients and the dose adjustment of teicoplanin, the clinical pharmacists make the clinical use of teicoplanin safer, more effective and more reasonable. |
