摘    要:目的 观察哮喘患者诱导痰中IL-13等细胞因子的浓度变化,探讨在哮喘发病与治疗过程中这些细胞因子的作用及其对哮喘治疗的影响。方法 27例重症哮喘患者随机分为单用激素(A)组、激素联用β2受体激动剂(B)组以及单用β2受体激动剂(C)组治疗,在治疗前及治疗开始后进行诱导痰标本采集,用ELISA法测定诱导痰中IL-13、IL-6、IL-10以及TNF-α浓度。结果 哮喘患者诱导痰中IL-13、IL-6、IL-10以及TNF-α浓度均高于对照组,A、B两组患者痰中IL-13浓度随着治疗进程、症状以及肺功能改善而显著降低。在治疗开始2周后,IL-6、IL-10浓度升高,TNF-α浓度降低,此后,没有出现显著性浓度变化。结论 IL-13有可能是哮喘发病中的关键因子,拮抗IL-13可能有助于减少激素治疗的剂量,抗IL-13治疗在哮喘治疗中的潜能不容忽视。

Abstract:Objective To observe the changes of concentrations of IL-13 and other cytokines in induced sputum from the patients with asthma,to discuss the actions of these cytokines,specially IL-13,and the effects of these cytokines on asthma exacerbation and processes of treatments. Methods Twenty-seven patients were grouped to A, B and C three groups, those were treated respectively with steroid therapy, steroid and beta-2 stimulator therapy cooperative and beta-2 stimulator therapy. Before and after the treatment,the samples of induced sputum were collected. And the measurements of IL-13, IL6, IL-10 and TNF-alpha were performed with ELISA kits later. Results The concentrations of IL-13, IL-6, ILl0 and TNF-alpha in induced sputum from the patients with asthma were higher than those of healthy subjects. In A and B groups,along with the processes of treatment and improvement of symptoms and lung function,the IL- 13 concentration fell significantly. After treatment beginning two weeks,the concentration of IL-6 and IL-10 increased, but TNF-alpha concentration decreased,after that, no significant changes were shown. Conclusions IL-13 may be a key cytokine in asthma exacerbation. It may help to decrease steroid dose in treatment of asthma by means of anti-IL-13. It indicated that potential of anti-IL-13 therapy for asthma must not be neglected.
