INSURE技术在新生儿呼吸窘迫综合征中应用的临床分析

摘    要:目的:研究INSURE技术(气管插管)在新生儿呼吸窘迫综合征中应用的有效性。方法将2011年1月至2013年9月入院的72例呼吸窘迫综合征患儿随机平均分为2组。实验组36例患儿采用INSURE技术治疗呼吸窘迫。对照组36例患儿采用常频通气(CMV)。比较两组患儿在呼吸功能、用氧时间上机后合并症及转归的情况。结果治疗后48 h 实验组患儿氧合指数相比对照组明显增高(P<0.05)。实验组患儿氧疗时间明显缩短(P<0.05),并且实验组患儿呼吸机相关肺炎(VAP)发生情况明显低于对照组(P<0.05)。两组患儿发生气胸、颅内出血、肺出血、支气管肺发育不良的发生率无差异(P>0.05)。结论 INSURE 技术能够更好地改善NRDS患儿呼吸氧合功能,减少呼吸机相关性肺炎(VAP)的发生率,并缩短了用氧时间,较CMV效果更好。

Abstract:Objective To study the effectiveness of insure technology in neonatal respiratory distress syndrome applications.Methods 72 cases of respiratory distress syndrome in children from January 201 1 to September 2013 were divided into two groups randomly and evenly .36 children in the experimental group used INSURE treatment of respiratory distress.36 children in the control group used CMV.The two groups of children were compared by respiratory function,time conditions with the oxygen machine after complications and outcomes Results 48 h after treatment in the experimental group compared to the control group of children with the oxygenation index was significantly higher (P<0.05 ).Oxygen therapy in the experimental group was significantly shorter(P<0.05 ).And the ventilator-associated pneumonia (VAP)of experimental group was significantly lower than the control group(P<0.05).Two groups of children pneumothorax,intracranial hemorrhage,pulmonary hemorrhage,bronchopulmonary dysplasia was no difference (P>0.05 ). Conclusion INSURE technology can better improve oxygenation in children of NRDS with respiratory function and reduce ventilator-associated pneumonia (VAP)incidence and shorten the time to use oxygen better than the CMV effect.
