盐酸达克罗宁对肛肠疾病术后的止痛效果分析

目的 探讨盐酸达克罗宁纱布对肛肠疾病术后的止痛效果.方法 选取进行肛肠手术的患者50例,采用随机数字表法分为盐酸达克罗宁组和亚甲蓝组,每组25例.观察组采用达克罗宁纱布术后包扎术后换药,对照组仅使用亚甲蓝进行术后止痛,在术后24 h到2周内观察止痛效果.结果 盐酸达克罗宁组的总有效率为92.0％（23/25）,明显高于亚甲蓝组的80.0％ （20/25）,差异有统计学意义（P＜0.05）.术后24 h,24 ～ 72 h及72 h～1周,盐酸达克罗宁组疼痛例数分别为10、1、0例,明显高于亚甲蓝组的22、16、5例（Х^2值分别为12.5,20.1,5.6,均P＜0.05）.盐酸达克罗宁组疼痛消失时间、水肿消退时间以及创面愈合时间分别为（5.4±1.2）h、（4.3±0.9）h、（14.4±3.7）h,明显低于亚甲蓝组的（8.9±2.7）h、（7.8±1.6）h、（18.9±4.2）h（t值分别为5.9,9.5,4.0,均P＜0.05）.结论 盐酸达克罗宁作为局部麻醉药进行肛肠术后止痛具有较好的止痛效果,且有效促进水肿消退及创面愈合.

Analgesic effect analysis of dyclonine hydrochloride on anorectal surgery

Objective To explore the analgesic effect of hydrochloric dyclonine gauze used during anorectal surgery.Methods Fifty patients with anorectal surgery received in our department were randomly divided into dyclonine hydrochloride group and methylene blue group with 25 cases in each group.The dyclonine hydrochloride group were treated with dyclonine hydrochloride gauze for postoperative dressing,the methylene blue group were treated with methylene blue for postoperative analgesic,the analgesic effect of 24 hours to a half months after surgery were observed.Results The total effective rate of dyclonine hydrochloride group was 92.0% （23/25）,which was significantly higher than that of methylene blue group 80.0% （20/25）],the difference was statistically significant （P 〈 0.05）.In postoperative 24 h,24-72 h and 72 h-1 week,the pain cases in dyclonine hydrochloride group were 10 cases,1 case,0 case,which were significantly higher than methylene blue group 22 cases,16 cases,5 cases（Х^2 =12.5,20.1,5.6,P 〈0.05）.The pain disappeared time,Edema subsiding swelling time and wound healing time in dyclonine hydrochloride group were （5.4 ± 1.2） h,（4.3 ± 0.9） h,（14.4 ± 3.7） h,which were significantly shorter than those in methylene blue group（8.9 ± 2.7） h,（7.8 ± 1.6） h,（18.9 ± 4.2） h,t =5.9,9.5,4.0,P 〈 0.05].Conclusion The dyclonine hydrochloride as a local anesthetic for anorectal postoperative analgesia has some advantages such as rapid onset,better analgesic effect and long-term cure rate.
