甲型和乙型流感确诊患者血浆C反应蛋白和血常规结果分析

摘    要:目的 探讨甲型和乙型流感确诊病例C反应蛋白(CRP)、WBC、中性粒细胞和中性粒细胞比率的变化及意义.方法 将534例发热门诊患者分为3组,甲型流感组207例、乙型流感组89例,238例.分别检测CRP和白细胞计数等指标并进行比较.结果 甲型流感组和乙型流感组WBC[(7.1±2.1)×109/L、(7.2±1.8)×109/L]、中性粒细胞计数[(5.2±1.9)×109/L、(5.3±1.5)×109/L]、中性粒细胞的比例(0.73±0.10、0.73±0.09)和CRP[( 15±8) mg/L、( 19±11) mg/L]均低于非流感组[分别为(8.8±3.9)G/L、(0.77±0.10)、(41±32) mg/L],差异均有统计学意义(均P〈0.05或P〈0.01);甲型流感组与乙型流感组之间差异无统计学意义(P〉0.05).甲型流感组和乙型流感组的体温水平与非流感组比较,差异无统计学意义(P〉0.05).甲型流感组和乙型流感组各项指标比较,差异无统计学意义(P〉0.05).3组在白细胞〉10×109/L 、中性粒细胞计数〉6.4×109/L 、中性粒细胞比〉0.75 、CRP〉10 mg/L各项中的构成比差异有统计学意义 [甲型流感组:10.6%(22/207),22.7%(47/207),45.4%(94/207),9.7%(20/207);乙型流感组:3.4%(3/89),21.3%(19/89),42.7%(38/89),18.0%(16/89);非流感组:23.1%(55/238),45.8%(109/238),61.3%(146/238),34.5%(82/238),均P〈0.05].结论 检测流感患者CRP和WBC的变化,对判断病情及指导临床用药均有一定的意义.

Abstract:Objective To investigate the changes of C-reactive protein(CRP), white blood cells count(WBC),neutrophil(NE), the percentage of neutrophils ( NE%) level in patients with influenza A and influenza B.Methods Five hundred and thirty four patients were divided into 3 groups as influenza A group(n=207)、influenza B group(n=89) and non-influenza group(n=238). C-reactive protein and white blood cells count were examined for all subjects. Results In influenza A group, the WBC and NE level were lower than those of non-influenza group (P〈0.01) ; the NE% and CRP level was lower than those of non-influenza group (P〈0.05) [NE%:(73±10)% vs (77±10)%,CRP:( 15±8) mg/L vs (41±32) mg/L]. In influenza B group, the WBC, NE, NE% and CRP level was lower than those of non-influenza group (P〈0.05) . In influenza A group and influenza B group, the temperature level was changed than those of non-influenza group (P〉0.05). In influenza A group, the WBC, NE, NE% and CRP level was changed than those of influenza B group(P〉0.05). There were significant differences in the constituent ratios in three groups [influenza A group:10.6%(22/207), 22.7%(47/207), 45.4%(94/207), 9.7%(20/207); influenza B group:3.4%(3/89), 21.3%(19/89),42.7%(38/89), 18.0%(16/89); non-influenza group: 23.1%(55/238), 45.8%(109/238), 61.3%(146/238), 34.5%(82/238), P〈0.05]. Conclusion Monitoring of C-reactive protein, white blood cells count level is helpful in diagnosis and the instruction clinical medication in patients with influenza.
