心脏彩色多普勒超声在新生儿先天性心脏病诊断中的应用价值

目的 分析和探讨彩色多普勒超声在临床中诊断胎儿及新生儿先天性心脏病的重要性和临床价值.方法 分析2010年11月至2012年11月在北京市怀柔区妇幼保健院出生的新生儿1 823例心脏彩色多普勒超声结果,以上新生儿均在孕期22～ 24周行胎儿超声心动检查.结果 在1 823例新生儿中,产前超声检出的复杂畸形先天性心脏病4例：主动脉弓离断1例,肺动脉狭窄1例,右心室双出口1例及单心室1例;新生儿先天性心脏病28例,室间隔缺损14例（50.0％）,其中膜部缺损10例（35.7％）,肌部缺损4例（14.2％）;房间隔缺损10例（35.7％）;不完全心内膜垫缺损1例（3.6％）;法洛四联症1例（3.6％）;动脉导管未闭2例（7.1％）.结论 彩色多普勒超声作为一种对胎儿及母体均无创的检查方法,对胎儿及新生儿心血管畸形的发现尤其是严重畸形的发现具有重要价值.

Application value of heart color Doppler ultrasound in the diagnosis of newborn congenital heart disease

Objective To analyze and discuss the significance and clinical value of color doppler ultrasound for the diagnosis of congenital heart disease（CHD）.Methods The reports of heart of color doppler ultrasound of 1 823 neonates in Beijing Huairou maternal and child health hospital between November 2010 and November 2012 were analyzed; all of neonates were given ultrasound examination in the 24-26 weeks of pregnancy.Results In 1 823 neonates,there were 4 cases with complex cardiac malformation,1 with interruption of aortic arch,1 with pulmonic stenosis,1 with double-outlet of right ventricle,1 with single ventricle,28 neonates with CHD,14 with ventricular septal defect （50.0%）,10 with atrial septal defect （35.7%）,1 case of endocardial cushion defect （3.6%）,1 with tetralogy of fallot （3.6%） and 2 with patent ductus arteriosus （7.1%）.Conclusion The color Doppler ultrasound is non-traumatic and it can effectively help the diagnosis of neonates with congenital heart disease,especially with serious cardiac anomalies.
