彩色多普勒超声在糖尿病患者颈动脉斑块检测中的应用价值

目的 探讨彩色多普勒超声在糖尿病患者颈动脉斑块检测中的应用价值.方法 收集2009年3月至2012年5月在解放军第四五七医院就诊并行彩色多普勒超声的糖尿病患者188例（糖尿病组）,另收集本院同期门诊体检健康者80例（健康组）,运用彩色多普勒超声检查入组者的双侧颈总动脉、颈内动脉、颈外动脉的内径、内膜-中膜厚度、斑块.结果 糖尿病组的颈总动脉内径大于健康组[（7.70 ±0.42） mm比（6.60 ±0.34） mm],2组比较差异均有统计学意义（P＜0.05）.糖尿病组颈动脉内-中膜厚度大于健康组[（1.03 ±0.11）mm比（0.70 ±0.20） mm],内膜面毛糙者多于健康组[122例（64.9％）比32例（40.0％）,差异有统计学意义（均P＜0.05）.糖尿病组斑块检出率大于健康组[76.1％ （143/188）比10.0％（8/80）],差异有统计学意义（P＜0.05）.结论 彩色多普勒超声可以为粥样硬化的早期预防和治疗提供客观依据,尤其对中老年的糖尿病患者在颈动脉检查中应用,具有较好的临床意义和实用价值.

Value of color Doppler ultrasound for carotid plague in patients with diabetes

Objective To observe the value of color Doppler ultrasound for carotid plague in patients with diabetes.Methods From March 2009 to May 2012,188 cases of diabetic patients were collected （diabetic group）.Eighty outpatient were collected （healthy group）.Bilateral carotid artery diameters,internal carotid artery diameter,the diameter of the external carotid artery intima-media thickness and plaque in both groups were compared.Results Carotid artery diameter and intima-media thickness in the diabetic group were larger than those in the healthy group [（7.70 ± 0.42） mmvs （6.60 ± 0.34） mm,（1.03±0.11） mmvs （0.70±0.20） mm]; Plaque detection rate in the diabetic group was higher than that in the healthy group [76.1% （143/188） vs 10.0% （8/80）] ; the difference was statistically significant （P 〈 0.05）.Conclusion Color Doppler ultrasound can provide an objective basis for early prevention and treatment of atherosclerosis sclerosis,especially in the carotid artery examination for elderly patients with diabetes.
