摘    要:目的探讨促红细胞生成素对实验性肝硬化大鼠动脉血浆内皮素-1的影响及意义。方法45只实验SD大鼠随机分成3组:肝硬化组(n=17)、r-HuEPO干预组(n=15)和正常对照组(n=13)。用四氯化碳制备肝硬化模型,成模后干预组予r-HuEPO干预2周。血浆内皮素-1含量采用放免法测定,自动血气分析仪测PaO2、PaCO2、AaDO2(肺泡-动脉血氧分压差)。结果肝硬化组血浆ET-1、PaO2、AaDO2含量与正常组、r-HuEPO干预组比较差异有显著性,正常组与r-HuEPO干预组间差异不显著,肝硬化组与r-HuEPO干预组动脉血浆ET-1及PaO2呈显著正相关(r=0.733,P<0.05;r=0.652,P<0.05),与AaDO2呈显著负相关(r=-0.755,P<0.01;r=-0.732,P<0.05),在其它两组无显著相关性。结论ET-1在肝硬化大鼠低氧血症发生机制中可能起重要作用,r-HuEPO可改善肝硬化低氧血症状态。

Abstract:Objective To study the alternation and the significance of endothelin-1(ET-1) in experimental rats with cirrhosis after using erythropoietin. Methods All the rats were divided into three groups: normal group, cirrhotic group and r-HuEPO group. Cirrhotic group were made by injection of of carbon tetrachloride, r-HuEPO group were treated with r-HuEPO after cirrhosis models were successfully constructed. The level of ET-1 in plasma was measured by RIA method.Results In cirrhotic group and r-HuEPO group rats, correlation analysis showed that a significant correlation was found between the phasma level of ET-1 and partial oxygen pressure of arterial blood(PaO_2), alveolar-arterial oxygen gradients(AaDO_2),respectively ((r=0.733,P<0.05;r=0.755,P<0.01;r=0.652,P<0.05;r=-0.732,P<0.05).The plasma level of ET-1 and PaO_2 were significantly lower in cirrhotic rats than those in both normal group and r-HuEPO group. In r-HuEPO group, the plasma level of ET-1 and PaO_2 were significantly higher than those in the other groups of cirrhotic rats. Conclusion ET-1 plays an important role in pathogenesis of hypoxemia in cirrhotic rats; r-HuEPO treatment could improve hypoxemia in cirrhotic rats by increasing the plasma level of ET-1.
