孟鲁司特对小儿反复呼吸道感染患者免疫指标及促炎因子的影响观察

目的:探讨孟鲁司特治疗小儿反复呼吸道感染的疗效。方法:将80例反复呼吸道感染患儿随机分成两组,对照组和观察组各40例。其中对照组在基础治疗的前提下肌注丙种球蛋白,100mg/次,15d一次,连续注射4次;观察组在此基础上口服孟鲁司特5mg/d,连续服用两个月。比较疗前及疗后两组间相关免疫指标,促炎因子及临床症状等差异。结果:治疗2个月后,各项免疫指标都得到了改善,其中观察组血清IgA、IgG1、IgG4含量显著高于对照组,痰液中EOS显著低于对照组,差异有统计学意义(P<0.05);疗后两组各项炎症因子指标都得到了不同程度的改善,其中观察组血清炎症因子CyslTs、IL-4、IL-5、TNF-α及IFN-γ均显著低于正常组,差异有统计学意义(P<0.05),血清IL-2也较正常组低;治疗后观察组有效率高达60%以上,而对照组仅为30%左右,观察组治愈率和显效率显著高于对照组,差异有统计学意义(P<0.05)。结论:孟鲁司特治疗小儿反复呼吸道感染能有效改善各项相关免疫指标和促炎因子,提高基础治疗的疗效。

Influence of montelukast on immune indices and inflammatory cytokines of children with recurrent respiratory infections

Objective:To explore the clinical effects of montelukast on children with recurrent respiratory infections.Methods:A Total of 80 children with recurrent respiratory infections were randomly divided into two groups,the control group and the observation group,each group had 40 cases.The children in control group were treated by gamma globulin,and those in observation group were treated by montelukast,5 mg/d,for two months.Then the levels of immune indices and inflammatory cytokines of two groups were analyzed and compared.Both the clinical effects of two groups were observed.Results:After two months,the levels of IgA,IgG1,IgG4 in serum of observation group were significantly higher than those of control group,and the percents of EOS in observation group was significantly lower than that of control group(P<0.05).The levels of serum inflammatory factor such as CyslTs,IL-4,IL-5,TNF-α and IFN-γ were significantly shorter than those of control group(P<0.05).Efficient rate of observation group was higher than 60%,however it was only 30% in control group,there were significant differences(P<0.05).Conclusions:The effects of montelukast on children recurrent respiratory infections are better than traditional herniorrhaphy,and it can significantly improve immune indices and inflammatory cytokines.
