不同透析方式对糖尿病肾病患者的心脏结构和功能的影响分析

目的:探讨糖尿病肾病(DN)患者采用不同透析方式对其心脏结构和功能的影响。方法:选取在本中心进行血透治疗的糖尿病肾病患者74例,依据透析方法不同分为观察组与对照组,观察组患者采用腹膜透析(PD)治疗,对照组采用血液透析(HD)治疗,定期对两组患者行超声心动图检查,同时测定两组患者血压、血容量及生化指标。结果:观察组血白蛋白、收缩压、超滤量明显低于对照组(P<0.05);观察组左室质量指数(LVMI)和左室肥厚(LVH)的发生率显著低于对照组(P<0.05),两组左室射血分数和E/A对比无统计学意义(P>0.05)。结论:观察组患者较对照组LVMI、LVH发生率低,表明PD可作为DN患者较理想的透析方式。

Analysis on effects of different dialysis methods on cardic structure and function among DN patients

Objective: To discuss effects of different dialysis methods on cardic structure and function among diabetic nephropathy(DN) patients.Methods: A total of 74 DN patients admitted into our center for dialysis were enrolled and divided into observation group and control group according to dialysis method: patients in observation group received peritoneal dialysis(PD) while those in control group adopted hemodialysis(HD).Ultrasonic cardiogram was performed,blood pressure,blood volume and biochemical indicators in two groups were measured regularly.Results: Blood albuminin,systolic pressure and ultrafiltration volume in observation group were significantly lower than those in control group(P<0.05).Left ventricular mass index(LVMI) and incidence rate of left ventricular hypertrophy(LVH) in observation group were significantly lower than that in control group(P<0.05).No significant differences were found between two groups in left ventricular ejection fraction or E/A(P>0.05).Conclusion: Observation group had lower incidence rates of LVMI and LVH than control group,indicating that PD could be adopted as an ideal dialysis method for DN patients.
