急性闭角型青光眼合并白内障不同治疗方式的疗效对比

目的:评估两种不同术式用于治疗急性闭角型青光眼合并白内障的临床疗效。方法:回顾性分析我院收治的120例急性闭角型青光眼合并白内障患者,随机分为A、B两组,A组60例(68眼)行白内障囊外摘除联合人工晶状体植入术,B组60例(67眼)行小梁切除术,比较两组患者术前、术后眼压、前房角开放程度及视力变化等指标。结果:A组患者的术后平均眼压为(13.35±1.96)mm Hg,前房角开放程度为(64.86±6.38)°,视力(0.76±0.35),而B组患者术后眼压平均为(24.44±2.46)mmHg,前房角开放程度(87.32±8.96)°,视力(0.52±0.19),A组患者术后各指标改善程度明显优于B组患者,差异有统计学意义(P<0.05)。A组术后并发症发生率为8.96%,B组并发症发生率为16.18%,A组术后并发症发生率明显低于B组,预后良好(P<0.05)。结论:白内障囊外摘除联合人工晶状体植入术治疗急性闭角型青光眼合并白内障疗效优于小梁切除术,术后并发症少,安全有效,值得临床推广。

Effect of different surgeries on treatment of acute angle-closure glaucoma and cataract

Objective:To investigate the effect of extracapsular cataract extraction and intraocular lens implantation and trabeculectomy on treatment of acute angle-closure glaucoma and cataract.Methods:A total of 120 cases patients with acute angle-closure glaucoma and cataract were randomly divided into group A and group B(each of 60 cases).Group A received extracapsular cataract extraction and intraocular lens implantation,and group B underwent trabeculectomy.Before and after surgery,visual acuity,angle of anterior chamber,intraocular pressure changes and the incidence of postoperative complication of two groups were compared.Results:After surgery,intraocular pressure in group A was(13.35±1.96) mm Hg,angle of anterior chamber in group A was(64.86±6.38)°,visual acuity in group A was(0.76±0.35),while,intraocular pressure in group B was(24.44±2.46) mm Hg,angle of anterior chamber in group B was(87.32±8.96)°,visual acuity in group B was(0.52±0.19),the difference was statistically significant(P<0.05).Complication rate in group A(8.96%) was lower than that in group B(16.18%),the difference was statistically significant(P< 0.05).Conclusions:Extracapsular cataract extraction and intraocular lens implantation in acute angle-closure glaucoma and cataract is safe and effective.
