2型糖尿病合并高血压患者尿微量白蛋白与颈动脉硬化的相关性分析

摘    要:目的:探讨2型糖尿病合并高血压患者尿微量白蛋白(U-MA)与颈动脉硬化的相关性,分析其临床意义。方法:选取2型糖尿病合并高血压患者共60例纳入观察组,并将单纯高血压患者、2型糖尿病患者各40例分别纳入高血压组和糖尿病组作为对照,检测并分析三组患者尿微量白蛋白(U-MA)水平及颈动脉粥样硬化情况。根据U-MA水平将观察组患者分为U-MA正常组与U-MA异常组,比较两组患者颈动脉内膜中层厚度(IMT)的差异,分析U-MA与IMT的相关性及其临床意义。结果:①观察组U-MA水平、U-MA异常比例、IMT厚度、斑块阳性例数均显著高于高血压组或糖尿病组(P<0.05)。②U-MA异常组IMT厚度显著高于U-MA正常组患者,并且该组患者视网膜病变(DR)、心血管疾病(CVD)、脑卒中等并发症发生率显著高于U-MA正常组(P<0.05)。③U-MA与IMT相关性具有统计学意义(β=0.75,P<0.01)。结论:2型糖尿病合并高血压患者U-MA异常的动脉粥样硬化阳性率较高,且U-MA与动脉粥样硬化存在一定的相关性,可用于防治DR、DN、CVD等相关并发症。

Relationship between uria micro-album and carotid atherosclerosis in type 2 diabetes mellitus complicated with essential hypertension
Abstract:Objective: To explore the relationship between uria micro-album(U-MA) and carotid atherosclerosis in type 2 diabetes mellitus(DM) complicated with essential hypertension(EH),and to analyze its clinical significance.Methods: A total of 60 DM patients complicated with EH were selected as observation group,while 40 DM patients and 40 EH patients were also enrolled as DM group and EH group.U-MA levels and carotid atherosclerosis of the three groups were detected and compared.Based on the level of U-MA,all patients in observation group were divided into U-MA normal group and U-MA abnormal group.Compare the carotid intima mediu thickness(IMT) between two groups and analyze the relationship between U-MA and carotid atherosclerosis.Results:1.The level of U-MA,percentage of U-MA abnormality,IMT and coronary atherosclerotic plaques in observation group was higher than that of EH group or DM group(P<0.05).2.IMT in U-MA abnormal group was higher than U-MA normal group,while the percentage of DR,CVD,cerebral apoplexy were higher than U-MA normal group.3.The level of U-MA has significant correlation with IMT(β=0.75,P<0.01).Conclusion:There was significant correlation between U-MA and IMT in patients of DM complicated with EH.U-MA and IMT can be used for predicting subclinical atherosclerosis and its complications.
