瑞芬太尼对颅脑手术患者脑血流及脑氧代谢的影响观察

摘    要:目的:分析瑞芬太尼对颅脑手术患者脑血流及脑氧代谢的影响。方法:将2010年6月~2012年12月的60例颅脑手术患者随机分为对照组(芬太尼组)30例和观察组(瑞芬太尼组)30例,然后将两组患者麻醉前及麻醉后10min、30min、停药后5min的大脑中动脉血流动力学指标、脑血管血液动力学指标及脑氧代谢指标进行比较。结果:观察组麻醉后10min、30min、停药后5min的大脑中动脉血流动力学指标波动幅度小于对照组,而脑血管血液动力学指标及脑氧代谢指标也均明显优于对照组,差异有统计学意义(P均<0.05)。结论:瑞芬太尼对颅脑手术患者脑血流及脑氧代谢的影响更为积极,对于此类患者的应用安全性相对更高。

Influence of remifentanil on the cerebral blood flow and cerebral oxygen metabolism in patients underwent craniocerebral operation
Abstract:Objective:To analyze the influence of remifentanil on cerebral blood flow and cerebral oxygen metabolism in patients with underwent craniocerebral operation.Methods:A total of 60 patients underwent craniocerebral operation from June 2010 to December 2012 were randomly divided into control group(fentanyl group,30 cases) and observation group(remifentanil group,30 cases),then the middle cerebral artery hemodynamics indexes,cerebrovascular hemodynamics indexes and cerebral oxygen metabolism of two groups before and 10 min,30 min after inducing anaesthesia and 5 min after withdrawal of after the anaesthesia were compared.Results:The fluctuation range of cerebral artery hemodynamics indexes of the observation group were lighter;cerebral artery hemodynamics indexes and cerebral artery hemodynamics indexes were superior than that of the control group showing significant difference between the two groups(P all<0.05).Conclusion:With great safety,remifentanil shows positive influence on the cerebral blood flow and cerebral oxygen metabolism indexes in patients underwent craniocerebral operation.
