单病种护理管理模式对严重烧伤瘢痕修复患者外观与功能恢复及心理应激反应的影响

目的:探究单病种护理管理模式对严重烧伤瘢痕修复患者皮肤外观、功能恢复及心理应激反应的影响。方法:将笔者医院创伤外科2018年1月-2019年7月收治的86例严重烧伤瘢痕修复患者随机分为观察组(43例)和对照组(43例)。观察组采用单病种护理管理模式,对照组采用常规护理。两组均护理至出院。比较两组干预前后心理评分、温哥华瘢痕量表(Vancouver scar scale,VSS)评分、疼痛视觉模拟(Visual analogue scale,VAS)评分以及健康调查简表(SF-36)评分;比较两组干预后瘢痕时间指标以及瘢痕修复疗效。结果:干预后,两组焦虑自评量表(Self-rating anxiety scale,SAS)和抑郁自评量表(Self-rating depression scale,SDS)、VSS、VAS评分均下降,且观察组显著低于对照组,差异有统计学意义(P<0.05)。干预后,两组SF-36各维度评分均升高,且观察组显著高于对照组,差异有统计学意义(P<0.05)。干预后,观察组愈合时间及皮肤血运恢复时间均短于对照组,差异有统计学意义(P<0.05)。干预后,观察组瘢痕修复总有效率(90.70%)高于对照组(72.09%),差异有统计学意义(P<0.05)。结论:应用单病种护理管理模式对严重烧伤瘢痕修复患者进行干预可促进瘢痕外观与皮肤功能的恢复,有助于缓解焦虑抑郁情绪,减轻患者疼痛,提高护理满意度及生活质量,疗效显著。

Effects of Single Disease Nursing Management Mode on Skin Appearance,Function Recovery and Psychological Stress Response in Patients with Severe Burn Scar Repair

Objective To explore the effect of nursing management mode of single disease on skin appearance,function recovery and psychological stress response of patients with severe burn scar repair.Methods Eighty-six patients with severe burn scars who were admitted to the department of trauma surgery from January 2018 to July 2019 were randomly divided into the observation group(43 cases)and the control group(43 cases).The observation group used a single disease care management mode,and the control group used routine care.Both groups were nursing to discharge.The psychological scores,the score of Vancouver scar scale(VSS),the score of pain visual analogue(VAS)and the score of the MOS item short from health survey(SF-36);The time index of scar and the effect of scar repair were compared between the two groups.Results After intervention,self-rating anxiety scale(SAS),self-rating depression scale(SDS),VSS and VAS scores of the two groups all decreased,and the observation group was significantly lower than those of the control group,the difference were statistically significant(P<0.05).After intervention,SF-36 scores in all dimensions were increased in two groups,and in the observation group were significantly higher than those in the control group(P<0.05).After intervention,the healing time and skin blood circulation recovery time of the observation group were shorter than those of the control group(P<0.05).After intervention,the total effective rate of scar repair of the observation group(90.70%)was higher than that of the control group(72.09%,P<0.05).Conclusion The application of single disease nursing management mode to the treatment of severe burn scar can promote the recovery of scar appearance and skin function,help to alleviate anxiety and depression,reduce pain,improve nursing satisfaction and quality of life,with significant effect.
