外用硅酮凝胶对烧伤后增生性瘢痕的疗效研究

目的:研究并分析外用硅酮凝胶对烧伤后增生性瘢痕的临床疗效。方法:选取2017年6月-2019年3月笔者医院收治的烧伤后增生性瘢痕患者123例,随机分为观察组(n=61)和对照组(n=62),两组均对瘢痕清洁消毒并佩戴弹力套,观察组予以硅酮凝胶外敷,对照组予以重组人表皮生长因子凝胶外敷。采用温哥华瘢痕量表(Vancouver scar scale,VSS)对瘢痕进行评分,采用李克特量表对临床症状进行评分。并调查患者满意度。结果:观察组总有效率95.08%高于对照组总有效率67.74%,差异有统计学意义(P<0.05)。治疗2个月后,观察组血管分布评分低于治疗前,瘢痕色泽评分、疼痛症状评分均低于治疗前且均低于对照组(P<0.05)。治疗4个月后,观察组瘢痕色泽、血管分布、柔软度评分,瘙痒、疼痛症状评分均低于治疗前,且均低于对照组(P<0.05)。治疗6个月后,观察组瘢痕色泽、血管分布、厚度、柔软度评分,瘙痒、疼痛、感觉异常症状评分均低于治疗前,且均低于对照组(P<0.05)。观察组满意率96.72%高于对照组满意率70.97%(P<0.05)。结论:外用硅酮凝胶可以明显改善烧伤后增生性瘢痕色泽、血管分布、厚度和柔软度等评分情况,减轻瘙痒、疼痛和感觉异常等临床症状,对烧伤后增生性瘢痕具有良好疗效。

Effect of External Silicone Gel on Hyperplastic Scar after Burn

Objective To analyze the curative effect of external silicone gel to patients with hyperplastic scar after burn.Methods Total of 123 patients with hyperplastic scar after burns admitted to our hospital from June 2017 to March 2019 were randomly divided into the observation group(n=61)and the control group(n=62).On the basis of cleaning and wearing elastic sleeve,the observation group was given silicone gel external application,while the control group was given recombinant human epidermal growth factor gel external application.Vancouver scar scale(VSS)was used to score the scars and likert scale was used to score the clinical symptoms.After the treatment,the patients'satisfaction was investigated.Results The total effective rate of the observation group was 95.08%,which was higher than that of the control group(67.74%,P<0.05).Two months after treatment,the score of vascular distribution in the observation group was lower than that before treatment,the scores of scar color and pain symptom in the observation group were lower than those before treatment,meanwhile lower than those in the control group(P<0.05).Four months after treatment,the scores of scar color,vascular distribution and softness,the scores of pruritus and pain symptoms in the observation group were lower than those before treatment,meanwhile lower than those in the control group(P<0.05).Six months after treatment,the scores of the scar color,vascular distribution,thickness and softness,the scores of pruritus,pain and paresthesia in the observation group were all lower than those before treatment,meanwhile lower than those in the control group(P<0.05).The satisfaction rate in the observation group was 96.72%which was higher than that in the control group(70.97%,P<0.05).Conclusion The external application of silicone gel can significantly improve the color,vascular distribution,thickness and softness of hyperplastic scar after burn,and also can reduce the clinical symptoms of pruritus,pain,paresthesia,which has a good curative effect on hyperplastic scar after burn.
