基于新型冠状病毒肺炎疫情形势下泌尿系结石危重患者的急诊处理策略

2019年12月以来我国武汉市爆发的新冠肺炎疫情目前已逐步发展成为蔓延至全球的爆发性流行性疾病。出于疫情防控需要,我国部分医院的择期外科手术暂停进行。而由泌尿系结石导致的危重患者,往往合并发热、感染性休克等症状,可能需要与新冠肺炎的相关临床表现进行鉴别;另外,在妊娠期肾绞痛合并发热患者中新冠肺炎相关筛查手段中有一部分运用受限,这些都给临床筛查和处理带来了挑战。陆军军医大学第二附属医院泌尿外科在2020年1月18日至2月19日救治了7例泌尿系结石危重患者,我们采用轻重分层模式、严格实施感染防控措施并遵照泌尿外科指南,使所有危重患者均手术顺利平稳出院。本文结合7例诊疗经验,探讨在疫情形势下开展此类临床救治工作的策略。

The emergency treatment strategy for urinary calculi during the epidemic of novel coronavirus pneumonia

The outbreak of novel coronavirus pneumonia(2019-nCoV) in Wuhan since Dec.2019 has gradually developed into a worldwide epidemic.Due to the need of epidemic prevention and control,surgical operations are suspended in some hospitals.Critical urolithiasis is often complicated with fever and septic shock and should be distinguished from clinical manifestations caused by novel coronavirus pneumonia.Meanwhile,renal colic during pregnancy complicated with fever poses great challenges to the screening of 2019-nCoV cases.From Jan.18,2020 to Feb.19,2020,7 patients with severe urinary calculi were admitted to our department.We adopted the stratification mode,followed urological guidelines,and implemented strict infection control measures.All operations were successful and the patients were discharged.This paper explores the strategy to carry out such clinical treatment under the epidemic situation.
