| 摘 要： | 目的 测定急性脑梗死(ACI)患者血浆中促肾上腺皮质激素(ACTH)和皮质醇水平及其昼夜节律的变化,探讨ACTH和皮质醇对ACI患者的临床意义.方法 采用放射免疫法检测34例病情程度不同的ACI患者和30例对照者在8:00、16:00和24:00时的血浆ACTH和皮质醇含量,分析24h内ACTH和皮质醇水平变化与神经功能缺损程度和预后的关系.结果 ACI患者8:00、16:00和24:00时的血浆ACTH和皮质醇水平均高于对照组(P<0.05);血浆ACTH和皮质醇水平在3个时点的变化与神经功能缺损程度呈正相关;神经功能缺损程度重、预后差的患者两种激素水平明显增高,昼夜节律逆转.结论 检测血浆ACTH和皮质醇水平,观察其昼夜节律变化,可判断ACI患者的病情和预后.
