下颌下腺上皮性肿瘤25例临床分析

目的探讨下颌下腺上皮性肿瘤的临床特征及诊疗要点。方法对25例经病理证实为下颌下腺上皮性肿瘤患者的临床资料进行回顾性分析。结果25例患者中，良性肿瘤13例，恶性肿瘤12例；发病平均年龄为61岁；男女为1．15：1。良性肿瘤主要为多形性腺瘤，而恶性肿瘤以腺样囊性癌最常见，其次是黏液表皮样癌。手术是良性肿瘤的惟一治疗方法，术后均没出现复发。大部分恶性肿瘤患者行手术和放疗相结合，术后3年生存率为75．0％；5年生存率为66．7％。结论良性肿瘤表现为缓慢生长的肿块，摘除肿瘤及下颌下腺是其安全有效的治疗方法。恶性肿瘤治疗应行根治术和术后放疗，预后主要取决于肿瘤的组织学类型和生物学特性。

Epithelial tumors of the submandibular gland:clinical analysis of 25 patients

Objective To explore the clinical characteristics and management approach of submandibular gland epithelial tumors. Methods The medical records of 25 patients with histologically confirmed epithelial tumors of the submandibular gland were retrospectively reviewed. Results In 25 cases, There were 13 benign tumors and 12 malignant tumors with a mean age of 61 years. The ratio of male to female was nearly 1.15 ： 1. Pleomorphie adenoma was the most frequently encountered benign tumor; while the commonest malignant tumor was adenoid cystic carcinoma, followed mueoepidermoid carcinoma. Surgery was the sole treatment for the benign tumors. There were no recurrences. In the majority of cases, patients with malignant tumors were treated with surgery and postoperative radiotherapy. The 3-year survival rate was 75.0% and 5-year survival rate was 66. 7%. Conclusion Benign tumors of submandibuiar gland manifest a mild course of disease, and local excision along with the gland is a safe and effective method of treatment. Radical surgery and postoperative radiotherapy is the treatment of the malignant tumors. Prognosis depends on the histopathology and biologic behavior of the specific type of malignant tumor.
